Using the Patient Generated Index to Evaluate Response Shift Post-stroke

Abstract: The PGI provides valuable information regarding changes in person's conceptualization of HRQL and values, but the feasibility of using an individualized measure during the first six months post-stroke is limited by the added complexity of completing and interpreting such a measure.

“…Using a variety of methods to identify response shift, 28%, 60%, and 78% of individuals at 6 months post stroke demonstrated response shift [3,25,26]. Reconceptualization response shift was suggested in 81% of individuals post denture treatment [5].…”

“…A major issue is that these methods are burdensome for both respondents and interviewers, affecting the cost and feasibility of a study. In addition, to yield meaningful information, these tests require a high degree of cognitive engagement on the part of the respondent [9][10][11].…”

“…Over the last 14 years the SEIQoL (and its shorter version, the SEIQoL-DW) and the PGI have been subjected to extensive testing across a range of clinical conditions in the UK, Australia and North America [28]. Psychometric properties of validity, reliability and responsiveness have been assessed for both instruments with variable results, although in general the findings have supported their use in clinical evaluations of health care interventions [1,10,27,34]. Neither instrument is appropriate for QoL assessment in a developing country and measures are mainly used in research, or occasionally planning and evaluating development interventions.…”

“…The Wellbeing in Developing Countries ESRC Research Group (WeD) is exploring the social and cultural construction of wellbeing in four developing countries (see http://www.welldev.org.uk for further information), which provided an opportunity to develop and validate the GPGI in a sizeable sample of respondents across three WeD countries (Ethiopia, Bangladesh, and Thailand) at differing stages of economic development 1 . The respondents were a representative selection of men and women of different ages, socio-economic statuses, and religions from urban, rural, and peri-urban areas.…”
