Evaluation of efficacy and safety of minimally invasive segmentectomy in the treatment of lung cancer

The aim of the study was to investigate the efficacy and safety of minimally invasive segmentectomy in the treatment of lung cancer. A total of 86 lung cancer patients in early stage were selected for the treatment between May, 2010 and December, 2010. The patients were randomly divided into the control (n=43) and observation (n=43) groups. Patients in the control group received conventional thoracotomy as treatment, while thoracoscopic segmentectomy was performed for patients in the observation group. Factors including intraoperative bleeding amount, number of dissected lymph nodes, surgery duration, postoperative intubation time and length of stay (LOS) were compared between the two groups. A visual analogue scale was used for comparison of the postoperative incisional pain experienced by patients in the two groups. The incidence rate of postoperative complications of patients in the two groups was observed. We also assayed variations in the levels of serum inflammatory factors C-reaction protein (CRP), interleukin (IL) −6 and −10 of patients prior to operation and on the 3rd, 5th and 7th days and after operation via ELISA, and on the 7th day after operation, we determined the pulmonary function of patients. During the 5-year follow-up, the recurrence and survival rate of patients in the two groups were observed. In the observation group, the intraoperative bleeding amount of patients was significantly lower than that in the control group, and the surgery duration, postoperative intubation time and LOS were all shorter than those in the control group (P<0.05). By contrast, no significant difference was detected in a comparison of the number of dissected lymph nodes of patients between the two groups (P>0.05). Additionally, in the observation group, patients suffered less pain after operation than those in the control group (P<0.05), and on the 3rd, 5th and 7th days after operation, the levels of CRP, IL-6 and −10 in the observation group were significantly lower than those in the control group (P<0.05). After operation, the incidence rate of complications in the observation group was significantly lower than that in the control group (P<0.05), and the recovery in pulmonary function after operation was superior to that in the control group (P<0.05). In addition, the 5-year survival rate of patients in the observation group was significantly higher than that in the control group, and the recurrence rate was significantly lower than that in the control group (P<0.05). Minimally invasive segmentectomy shows better efficacy in the treatment of lung cancer at early stage than the conventional thoracotomy. In addition to the high safety during surgery, this technique can lower the incidence rate of postoperative complications, protect the pulmonary function, increase the survival rate and decrease the recurrence rate, which shows great value in clinical practice.

This study sought to investigate the analgesic effects of nalbuphine on patients undergoing thoracoscopic lobectomy during the perioperative period, as well as its effects on inflammatory cytokines. We selected 92 patients with early lung cancer who were admitted to Xiangyang No. 1 People's Hospital between January 2016 and December 2016. The patients were randomly divided into control and observation groups (n=46 each). All patients underwent thoracoscopic lobectomy, with those in the observation group receiving intravenous nalbuphine hydrochloride prior to induction (the control group received saline). Intraoperative blood loss, operation time and anesthetic dosages were compared between groups. The analgesic effects during the perioperative period were compared using a visual analogue scale. The adverse effects of anesthetics (nausea, vomiting, dizziness and drowsiness) were compared between the two groups. Finally, serum inflammatory cytokine [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and IL-10] levels were measured using ELISA at 1, 3, 5 and 7 days after operation. There were no significant differences in intraoperative blood loss and operation time between the two groups (p>0.05). However, dosages of propofol and remifentanil used were lower in the observation group than in the control group (p<0.05), and the effective rate of postoperative analgesia in the observation group was significantly higher than that in the control group (p<0.05). The degree of postoperative pain in both groups were lower than preoperative rates (p<0.05), while the incidence of adverse reactions in the observation group was significantly lower than in the control group (p<0.05). Finally, TNF-α, IL-6 and IL-10 levels were significantly lower in the observation group relative to the control group at 1, 3, 5 and 7 days after operation (p<0.05). Therefore, nalbuphine has a significant analgesic effect during thoracoscopic lobectomy. The application of nalbuphine can reduce the incidence of adverse reactions, reduce postoperative inflammatory responses, and promote rapid patient recovery, thus demonstrating the clinical value of nalbuphine.

Background: For tracheobronchial diseases, fiberoptic bronchoscopy (FOB) continues to be the gold standard diagnosing technique. It is a rather secure diagnostic and therapeutic method. Because of its excellent sensitivity, fiberoptic bronchoscopy is now the primary diagnostic method for assessing tracheobronchial endoluminal and mucosal diseases. Aim of The Work:To compare virtual bronchoscopy with fiberoptic bronchoscopy as the gold standard tool in diagnosis of cases of hemoptysis and identifying tracheobronchial disorders in patients having clinical and/or radiological reasons for bronchoscopy. Patients and Methods: Patients having hemoptysis with or without CXR or conventional CT abnormalities. Patients with high creatinine level, lactating and pregnant women were excluded from the study. Results: For the research, 50 patients were qualified. There were 50 research participants, 36 of whom were men (72%) and 14 of them were women (28%). The research group's average age was 59, with a standard deviation ± 14. Of the 50 patients, 42 patients had tracheobronchial abnormalities on fiberoptic broncoscopic examination, and virtual bronchoscopic examination confirmed the same abnormality in 37 (88%) patients and did not reveal any abnormality in 5 (12%) patients. an 8 of the 50 patients had no tracheobronchial abnormalities by either diagnostic method. The majority of lesions were obstructive (59.5%), nonobstructive lesions accounted for 23.8%, and partially obstructive lesions accounted for only 16.6%. Regarding the ability to bypass the lesion, out of 42 patients in whom FOB showed a positive lesion, only 15 lesions (35.8%) could be bypassed, while 100% of the lesions detected with VB could be bypassed. Based on the data collected and analysed, the sensitivity of virtual bronchoscopic examination was estimated to be 88% and the specificity was 100%. The PPV and NPV were calculated to be 100% and 61.5%, respectively, and the overall accuracy was estimated to be 90%. Conclusion:Virtual bronchoscopy is an innovative, non-invasive, accurate and uncomplicated diagnostic modality in the diagnosis of tracheobronchial diseases. In particular situations, VB could be the modality of choice, as in the assessment of the airways distal to strictures or stenosis.
