BackgroundDiagrammatic recording of finger joint angles by using two criss-crossed paper strips can be a quick substitute to the standard goniometry. As a preliminary step toward clinical validation of the diagrammatic technique, the current study employed healthy subjects and non-professional raters to explore whether reliability estimates of the diagrammatic goniometry are comparable with those of the standard procedure.MethodsThe study included two procedurally different parts, which were replicated by assigning 24 medical students to act interchangeably as 12 subjects and 12 raters. A larger component of the study was designed to compare goniometers side-by-side in measurement of finger joint angles varying from subject to subject. In the rest of the study, the instruments were compared by parallel evaluations of joint angles similar for all subjects in a situation of simulated change of joint range of motion over time. The subjects used special guides to position the joints of their left ring finger at varying angles of flexion and extension. The obtained diagrams of joint angles were converted to numerical values by computerized measurements. The statistical approaches included calculation of appropriate intraclass correlation coefficients, standard errors of measurements, proportions of measurement differences of 5 or less degrees, and significant differences between paired observations.ResultsReliability estimates were similar for both goniometers. Intra-rater and inter-rater intraclass correlation coefficients ranged from 0.69 to 0.93. The corresponding standard errors of measurements ranged from 2.4 to 4.9 degrees. Repeated measurements of a considerable number of raters fell within clinically non-meaningful 5 degrees of each other in proportions comparable with a criterion value of 0.95. Data collected with both instruments could be similarly interpreted in a simulated situation of change of joint range of motion over time.ConclusionsThe paper goniometer and the standard goniometer can be used interchangeably by non-professional raters for evaluation of normal finger joints. The obtained results warrant further research to assess clinical performance of the paper strip technique.

Nada a declarar RESUMO: Existe uma grande variedade de instrumentos utilizados para a avaliação da força de preensão manual. Porém, não existem estudos demonstrando a validade e a confiabilidade da maioria destes instrumentos. O objetivo deste estudo foi avaliar a validade concorrente e a confiabilidade intraexaminador do dinamômetro Saehan hidráulico comparado-o com o dinamômetro Jamar hidráulico. Cem indivíduos sadios (50 homens e 50 mulheres), entre 20 e 50 anos de idade, sem alterações cognitivas, deficiências físicas, disfunções neuromusculares e ortopédicas e história de lesões nos membros superiores, foram testados com os dinamômetros Jamar e Saehan. A validade concorrente entre o dinamômetro Jamar e o dinamômetro Saehan foi excelente para os testes de força de preensão realizados com as mãos direita (r=0,976) e esquerda (r=0,986). A confiabilidade intra-examinador foi excelente tanto para o dinamômetro Jamar (r=0,985 mãos direita e esquerda) quanto para o dinamômetro Saehan (r=0,981 mão direita e r=0,985 mão esquerda). O dinamômetro Saehan é válido, confiável e comparável com o dinamômetro Jamar. Portanto, dados coletados com o dinamômetro Jamar são equivalentes aos dados coletados com o dinamômetro Saehan. Consequentemente, valores de força obtidos por testes com dinamômetro Saehan podem ser comparados com valores de referência de força de preensão que foram obtidos com o dinamômetro Jamar. DEScRitORES: dinamômetro; força de preensão manual; validade; confiabilidade.ABStRAct: There is a great variety of instruments available for evaluating hand grip strength. There is, however, a lack of studies showing the validity and reliability of most of these instruments. The purpose of this study was to examine the concurrent validity and test-retest reliability of the hydraulic Saehan dynamometer using the hydraulic Jamar dynamometer. One hundred healthy subjects (50 men and 50 women), between the ages of 20 and 50 years old, without cognitive impairment, physical disability, neuromuscular or orthopedic dysfunction, and history of upper extremity injuries were tested with the Jamar and Saehan dynamometers. The concurrent validity between the Jamar and the Saehan dynamometers was excellent for both the right hand (r=0.976) and the left hand (r=0.986). Test-retest reliability was excellent for both the Jamar (r=0.985 right and left hands) and the Saehan (r=0.981 right hand and r=0.985 left hand). The Saehan dynamometer is valid, reliable and comparable to the Jamar dynamometer. Therefore, the Jamar and Saehan dynamometers measure handgrip strength equivalently. As a result, strength values obtained with the Saehan dynamometer can be compared with reference values which were collected with the Jamar dynamometer.

[Purpose] The aim of this study was to examine the test-retest reliability of pinch strength testing in elderly subjects with thumb CMC OA. [Subjects and Methods] A total of 27 patients with unilateral right-thumb CMC OA (mean ± SD age: 81.3 ± 4.7 years) were recruited. Each patient performed three pain-free maximal isometric contractions on each hand on two occasions, one week apart. Three different measurements were taken: tip, tripod, and key pinch strength. Intraclass correlation coefficient (ICC), standard error of measurement (SEM), and 95% limits of agreement (LOA) calculations were performed. [Results] Test-retest reliability of measurements of tip, tripod, and key pinch strength was excellent for the affected side (ICC=0.93, 0.96, and 0.99) and the contralateral thumb (ICC=0.91, 0.92, and 0.94). [Conclusions] The present results indicate that maximum pinch strength can be measured reliably using the Pinch Gauge Dynamometer, in patients with thumb CMC OA, which enables its use in research and in the clinic to determine the effect of interventions on improving pinch strength.
