Value of Special Auditory Tests in the Latero-Medial Inferior Pontine Syndrome

Abstract: Results from a special battery of auditory tests are used to document, for the first time, recovery from a total unilateral hearing loss occurring as part of a latero-medial inferior pontine syndrome. Although sensitivity for pure tones and speech was regained within two months of onset, definite retrocochlear signs persisted for nine months. The tests which best demonstrated the lingering retrocochlear abnormality were binaural masking level difference procedures for 500 Hz pure tones and speech and examinati… Show more

“…[1][2][3][4][5][6][7][8] Several prior studies 9-11,18 -21 of AICA infarction have described acute hearing symptoms. However, most were case reports, 9,11,18,19,21 and there has been no systematic study of a consecutive series of patients. In 1990, Hinojosa and Kohut 9 reported a clinicopathological study in a patient with complete AICA infarction.…”

“…Since the MS group yields small MLD and the cortical lesion group normal MLD, an obvious speculative conclusion is that MLD may have unique value in detection of brain stem and/or midbrain lesions. The recent report of Noffsinger et al 9 showing reduced MLD for a patient having a lateromedial inferior pontine syndrome lends further credence to such a supposition.…”

“…In addition to various batteries measuring central auditory function, another innovative technique for detecting central auditory brainstem pathology is a psychoacoustic phenomenon known as masking level difference (MLD). Masking level differences have been shown to demonstrate the continuing presence of auditory brainstem lesions despite normal conventional pure-tone and speech sensitivity tests (Quaranta and Cervellera, 1974;Noffsinger et al, 1975;and Olsen et al, 1976).…”
