Variability in electrodiagnostic findings associated with neurogenic thoracic outlet syndrome

Abstract: Introduction/Aims Neurogenic thoracic outlet syndrome (NTOS) is a heterogeneous and often disputed entity. An electrodiagnostic pattern of T1 > C8 axon involvement is considered characteristic for the diagnosis of NTOS. However, since the advent of high‐resolution nerve ultrasound (US) imaging, we have encountered several patients with a proven entrapment of the lower brachial plexus who showed a different, variable electrodiagnostic pattern. Methods In this retrospective case series, 14 patients with an NTOS … Show more

“…One article was excluded because it presented results of pre-surgery electrophysiological evaluations [ 27 ]. Finally, we included nine articles [ 14 , 17 , 28 , 29 , 30 , 31 , 32 , 33 , 34 ] ( Figure 1 ). Three studies were eligible for meta-analysis [ 28 , 29 , 32 ].…”

“…Mul et al report that imaging was able to capture a wide range of nTOS severity, detecting structural abnormalities as mentioned above 16 . They also note that it was complementary to EDX, favoring a diagnosis of nTOS in half of their cases.…”

“…In this issue of the Journal, Mul et al retrospectively reviewed the diagnostic data of 14 patients with nTOS from two Dutch centers 16 . They found that in a considerable portion of their patients the electrodiagnostic (EDX) abnormalities did not adhere to the previously described classic patterns, with apparently preserved compound muscle action potentials in thenar muscles and sensory nerve action potentials of the medial cutaneous antebrachial nerves, abnormal median sensory responses, or even abnormal needle electromyography (EMG) proximal to the thenar muscles.…”
