Variant of the extensor pollicis tertius: a case report on a unique extensor muscle to the thumb

Abstract: Variations on muscular and tendinous connections of the hand occur frequently in the human population and are often discovered during routine surgical procedures and cadaveric dissections. A knowledge of such anomalies is important to the physician in order to avoid unintentional damage to healthy tendons during surgical procedures. In addition, accessory tendons have the potential to be used in the repair or replacement of damaged tendons through surgical transfer or transplantation. Here we describe a unique… Show more

“…The knowledge of the variations in the origin of its nerve supply is important while this muscle is being harvested Besides, the tendon of the extensor muscles is very superficial and is frequently injury than the tendons of flexor muscles [16]. Accessory tendons have the potential to be used in the repair or replacement of damaged tendons through surgical transfer or transplantation [5]. Anomalies of muscles and tendons in hand and wrist are significant during hand surgery.…”

“…Successful tenosynovectomy in the treatment of de Quervain's disease requires an adequate anatomical knowledge of extensor tendons [1,4]. “…Anatomical variations of the finger extensor tendons are not uncommon and have been described by several authors (ABDEL-HAMID, EL-BESHBISHY and ABDEL AAL, 2013; BETTENCOURT PIRES, CASAL, MASCARENHAS DE LEMOS et al, 2013;BLUTH, WU, ELENA STARK et al, 2011;CASANOVA MARTINEZ, VALDIVIA GANDUR and GOLANO, 2013;CASANOVA-MARTINEZ, VALDIVIA-GANDUR and GOLANO, 2014;AVDAR and SEHIRLI, 1996;LI and MAO, 2014;MEHTA et al, 2009;NAYAK, HUSSEIN, KRISHNAMURTHY et al, 2009;NAYAK, KRISHNAMURTHY, PAI et al, 2008;NAYAR and MCARTHUR, 2009;RAY, RANADE, D'COSTA et al, 2010;ROY, MEHTA, SURI et al, 2010;YAMMINE, 2014YAMMINE, , 2015ZAINO, MITGANG, RAWAT et al, 2014). Some of these studies included anatomical variations of the first dorsal compartment of the wrist in relation to their importance in de Quervain's disease (NAYAK, HUSSEIN, KRISHNAMURTHY et al, 2009;ROY, ROY, DE et al, 2012;ROY, MEHTA, SURI et al, 2010).…”

“…The clinical significance of this abnormality is easily overlooked considering that most variations are discovered incidentally during surgery or cadaveric and radiologic studies. 2 , 4 , 5 , 6 , 7 , 8 , 9 , 10 Few cases have presented with symptomatic EPL variations. Regardless of preoperative symptoms, knowledge of these abnormalities is imperative to avoid complications in dorsal plating of the radius.…”
