Vasculitis with superior ophthalmic vein thrombosis compatible with neuro-neutrophilic disease

Abstract: PurposeTo present a unique case of neuro-neutrophilic disease with inflammation and thrombosis of the superior ophthalmic vein (SOV).ObservationsA 43-year-old Japanese man with past histories of oculomotor paralysis, auditory disorder, ischemic enteritis, and recurrent oral ulceration was referred to our hospital because of blurred vision in his right eye. Ophthalmic examination revealed decreased best corrected visual acuity and central scotoma in his right eye. Orbit magnetic resonance imaging (MRI) revealed… Show more

“…In this study, we detected a case of SOVT because of hypercoagulability state secondary to long lasting elevated factor VIII. Inflammatory diseases (Graves’ disease, systemic lupus erythematosus, sarcoidosis, Behçet syndrome, amyloidosis, vasculitis and ulcerative colitis) have been reported to be associated with the SOVT [[4], [5], [6],12]. We detected SOVT in a case with systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA) and a case of Sjogren's syndrome.…”

“…Periphlebitis of the superior ophthalmic vein (SOV) is an uncommon radiographic finding that can occur in various specific and nonspecific orbital inflammatory syndromes, including TED. The syndrome is a poorly defined entity, and inflammation of the SOV has been described as vasculitis, 1 partially occlusive SOV thrombosis, 2 venous congestion, 3,4 and periphlebitis. 5 The key characteristics of periphlebitis relate to inflammatory features surrounding the venous system with a patent vascular lumen.…”

“…Regarding the systemic factors involved, ophthalmic vein thrombosis has been associated with a variety of conditions such as hypercoagulability (thrombocytosis, antiphospholipid syndrome, hereditary hemorrhagic telangiectasia, use of contraceptive pills or pregnancy) [ 8 ]. Some studies in the literature describe the association of SOVT with other systemic inflammatory diseases (Graves’ disease, systemic lupus erythematosus, sarcoidosis, amyloidosis or vasculitis) [ 2 , 4 , 9 , 10 , 11 , 12 , 13 ].…”
