Multifocal and toric intraocular lenses (IOLs) or the so-called premium IOLs are currently widely used in adult patients as a one-step refractive solution following cataract surgery. However, the decision to implant a premium IOL in a pediatric patient involves multiple factors affecting the child’s visual development and is associated with several dilemmas and surgical challenges. The purpose of this review is to summarize these factors and analyse the influence of each of them on the visual outcomes following premium IOL implantation. A review of literature was conducted using the relevant keywords from various databases until 31st January 2022. All pertinent studies with multifocal or toric IOL implantation in children were reviewed, and relevant articles were studied in detail for age, IOL power calculation, visual outcomes (refractive outcomes, contrast sensitivity and stereopsis) and complications such as dysphotic phenomena and others. A total of 17 relevant studies (10 case series/interventional studies and 7 case reports) on the subject were included. All studies showed a favourable refractive outcome; however, the data available was significantly less. Studies with earlier models of multifocal IOLs showed a higher incidence of IOL decentration and posterior capsule opacification; however, more recent studies with newer IOL models showed much better safety profiles. Toric IOLs showed promising results in all the studies evaluated. Premium IOLs have shown promising results in the pediatric age group. However, their long-term outcomes specifically concerning refractive shift, capsular contraction and role in the management of amblyopia needs to be explored further.

Cataract surgery in children is more difficult and the rate of complications is higher when compared to adult surgery. The rate of intraoperative and postoperative complications is inversely proportional to the age at surgery. In addition to age, surgical technique can influence the rate of complications (e.g., primary intraocular lens implantation vs. aphakia). Higher intraoperative complications in children may be attributed to the common occurrence of an immature iris that results in poor pupil dilation. In addition, the eyes of young children are small and soft. Even small and well-constructed surgical wounds leak when not sutured securely. The anterior capsule in children is highly elastic and difficult to predictably tear into a circular and centered capsulorhexis. Scleral collapse and a formed, non-liquified, vitreous body create the upthrust commonly referred to as posterior vitreous pressure. Postoperative complications are also higher due to an intense inflammatory response and poor compliance to postoperative topical anti-inflammatory drops. The detection of postoperative complications can be challenging because of the difficulties encountered in performing detailed postoperative examinations in children, who may be uncooperative.

The greatest concern in children with cataracts is irreversible visual loss. The timing of congenital cataract surgery is critical for the visual rehabilitation. Cataract surgery in children remains complex and challenging. The incidence of complications during or after operation is higher in children than adults. Some complications could be avoided by meticulous attention to surgical technique and postoperative care, and others were caused by more exuberant inflammatory response associated with surgery on an immature eye or the intrinsic eyes abnormalities. Utilizing of advanced techniques and timely applying topical corticosteroids and cycloplegic agents can reduce the occurrence of visual axis opacification. Operation on children with strabismus or nystagmus, and applying occlusion therapy on amblyopic eyes can balance the visual inputs to the two eyes. Diagnosis of glaucoma following congenital cataract surgery requires lifelong surveillance and continuous assessment of the problem. So cataract surgeries in children are not the end of journey, but one step on the long road to visual rehabilitation. This paper describes recent evidence from the literature regarding the advance of management after congenital cataract surgery.

Abstract: Children- who underwent congenital cataract surgery by different surgical techniques showed a significant spontaneous reduction in DEQ values. A significant change in cylinder vector and SEQ was found after operations using scleral tunnel technique.
