Objectives: To report the safety and efficacy results of a 9- to 15-year follow-up investigation among patients who had received Carpentier-Edwards Perimount (CE-P) bovine pericardial bioprostheses (Edwards Lifesciences, Irvine, CA, USA) for valve replacement. Methods: This retrospective study investigated freedom from structural valve deterioration (SVD) as well as survival and reoperation among different age and etiology groups in patients who were implanted with a CE-P bioprosthesis at Guangdong General Hospital between 2001 and 2007. Kaplan-Meier survival analysis and multivariate Cox proportional hazards regression were performed. Results: The mean age of the patients (N = 225) was only 61.2 ± 11.5 years at valve replacement. More than half of the patients (55.1%) had rheumatic heart disease. The survival rates were 86.46, 81.58, and 74.42% at 5 years, 64.39, 66.19, and 55.85% at 10 years, and 48.37, 57.33, and 46.54% at 15 years for the groups with mitral valve replacement (MVR), aortic valve replacement (AVR), and double valve replacement (DVR), respectively. The median time to freedom from SVD was 12.5, 13.2, and 11.2 years, respectively, for patients with MVR, AVR, and DVR. A higher age at valve replacement was a significant risk factor for SVD in all patients (p < 0.01). Conclusions: Good long-term clinical results of CE-P valves have been demonstrated in Chinese patients >60 years.

ПРИОБРЕТЕННЫЕ ПОРОКИ СЕРДЦАСовременные общемировые тенденции в области хи-рургического лечения приобретенных пороков сердца де-монстрируют значительное увеличение количества им-плантаций биологических клапанов по отношению к ис-пользованию механических протезов [1,12]. Данное об-стоятельство связано с изменениями в структуре пороков и соответственно увеличением среднего возраста опери-руемых пациентов, а также возможностью обеспечения лучшего качества их жизни за счет отказа от перманентно-го использования антикоагулянтной терапии при условии Объем наблюдения -213,9 пациенто-лет. Почти в 6% случаев оперативные вмешательства носили повторный характер, в 80% -были сочетанными. Результаты. Линеаризованный показатель отдаленной выживаемости составил 3% на пациента в год. Актуарные показатели общей и кардиальной выживаемости к 5-му году наблюдения составили 84,7 и 97,8% соответственно. Актуарный показатель отсутствия реопераций к 5-му году был равен 98,5%, отсутствие протезного эндокардита наблюдалось в 98,5% случаев. Госпитальная летальность при реоперациях отсутствовала. Случаев первичной тканевой несостоятельности, тромбоэмболий и кровотечений в течение 5 лет наблюдения выявлено не было. Выводы. Применение биологического протеза ЮниЛайн у пациентов 65 лет и старше обеспечивает высокую выживаемость к 5-му году наблюдения, сопоставимую с результатами применения современных зарубежных моделей биологических протезов клапанов сердца. Биологический протез ЮниЛайн имеет высокую толерантность к инфекциям. Имплантация биопротеза ЮниЛайн приводит к полноценной коррекции внутрисердечной гемодинамики: уменьшению размера левого предсердия и снижению систолического давления в легочной артерии при наличии синусового ритма. Даже частичное сохранение подклапанных структур достоверно улучшает течение процесса ремоделирования миокарда левого желудочка при коррекции митрального порока. Ключевые слова: биологический протез клапана сердца, митральный порок.Aim. To estimate immediate results of mitral valve replacement for acquired valve disease using Uniline bioprosthesis. Material and methods. Since January 2009 to September 2014 in the Research Institute for Complex Issues of Cardiovascular Diseases (Kemerovo) Uniline bioprosthesis was implanted into mitral position in 85 patients (mean age 64.8±8.5 years). Average follow-up was 2.6±1.8 years (range 0.2-5.4 years). Observation's volume is 213.9 patient-years. Repeated and combined interventions were performed in almost 6 and 80% of cases respectively. Results. Linearized long-term survival was 3% patient-years. Actual overall and cardiac 5-year survival were 84.7 and 97.8% respectively. Actual 5-year reoperation-free index is 98.5%, 5-year absence of prosthetic endocarditis was observed in 98.5% of cases. There was no in-hospital mortality in case of reoperations. Primary tissue failure, thromboembolism and bleedings have not been observed for 5-year follow-up. Conclusions. 1.
