Vestibular function in Lermoyez syndrome at attack

Abstract: Lermoyez syndrome (LS) has been regarded as a variant of Ménière's disease (MD), but so far there have been very few cases of LS reported in the literature, so such a conclusion is debatable. Specifying the pattern of auditory and vestibular changes at attack using objective quantitative measures is important for understanding the mechanism responsible for MD and LS. Here we report the first objective measures of dynamic otolith function and dynamic semicircular canal function in an LS patient at the time of t… Show more

“…The characteristic clinical sign—the VVOR deficit—is usually simple to elicit and in most patients, special investigations are not required. The deficit in the VVOR and in the VOR can now be easily identified, documented, and measured with video‐oculography (VOG) 10–12 . Diagnosing CANVAS correctly and differentiating it from similar but more aggressive acquired (e.g., multiple system atrophy of cerebellar type [MSA C ] or Creutzfeldt‐Jakob disease with cerebellar impairment) and inherited diseases (e.g., Machado‐Joseph disease/spinocerebellar ataxia type 3 [SCA 3]) is important for patient management.…”

“…Manzari et al 93 reported that reduced oVEMPs and stable cVEMPs were observed in Lermoyez's syndrome, opposite to augmented oVEMPs and reduced cVEMPs during Meniere attacks, providing physiological evidence to support that Lermoyez's syndrome is a variant of Meniere's disease.…”

“…The simplicity of HIMPs and its relatively innocuous character for patients means that it can be used even during vertigo attacks. So it has been used to show in patients with Meni ere's disease how semicircular canal function fluctuates during and around the time of the attack of vertigo [34,35]. It has also been used to help discriminate between the vertigo due to Meni ere's disease as opposed to vertigo due to peripheral vestibular neuritis [36], as well as documenting the full recovery after peripheral vestibular neuritis [37].…”
