The principle of a suction cup is used to retain an upper full denture. Alveolar resorption results in loss of the vestibular depth and in diminished functional surface to overcome lateral forces. Traditional surgery has the following characteristics in common-need for incisions with steel scalpels, mechanical excision of the muscle attachment from the periosteum, placement of sutures, significant postoperative discomfort, and delayed healing. When the benefits and risks of alternative treatment modalities are taken into account for this mechanical surgical procedure, minimally invasive laserassisted soft-tissue excision and ablation seems to be very attractive technique. In the presented case report we used an Er:YAG laser (LiteTouch, Syneron, Israel) to deep the oral vestibulum of upper jaw in edentulous patient. Therefore, were indicated, we recommend the laser assisted operation technique for the partial vestibuloplasty to assure the patients a better postoperative outcome.

Surgical vestibular deepening procedures are well established and are being routinely performed.In spite of being an old technique, postoperative results in most of the instances are disappointing. Usually eventual loss of the clinically gained vestibular depth obtained through the surgical procedures occurs. This happens mainly due to rapid epithelialisation of the surgical wound and healing by scarring. Therefore, in the present case, after the initial incision, the labial free end of flap was sutured by a new technique so as to prevent rapid epithelialisation. Finally, the incision was completed and vestibular deepening was achieved with the help of diode laser as they are reported to produce remarkably less scarring at the surgical site.

Today more and more patients with bleeding disorder come to our office for treatment. The number of patients with therapeutic anticoagulation is growing steadily. Discontinuation of this therapy can often be crucial. On the other hand are oralsurgical procedures extremely sensitive to bleeding due to the constant presents of saliva with its high fibrinolytic activity.The aim of this study was to evaluate the use of a surgical diode ( wavelength 8 1 0 urn) laser as accessory tool in the treatment ofpatients with hemorrhagic diathesis. Enclosed in the study were 123 patients with:. Coumarintherapy, a Quick test ratio of I 5-25% . More than 300mg ofASS /die . Hemophilia, a factor activity under 35% . Morbus Werihof with less than 30000 thrombocytes 179 Surgical procedures: . Tooth extraction 86% . Apexectomy 3% . Tumorexcision 9%. Curettage and flapsurgery 1% . Gingivectomy 1%All procedures were laser-assisted with a diode laser emitting 8 1 Onm. The glass fibers used were depending on the procedures either 200, 400 or 600micron. No coagulating agents or tissue adhesives were used in addition. The postoperative outcome and complication rates were compared to substitution therapy and tissue adhesives. Laserassisted treatment ofthese patients shows a high predictability and success rate leaving out side effects of drugs and human cryoprecipitates. Postoperative impairment is diminished.
