Viable versus nonviable positive margins in Ewing sarcoma and associated recurrence rates: A systematic review

Abstract: Residual tumor after curative intent therapy in patients with Ewing's sarcoma is of great clinical significance. Surgeons use the resection margin to indicate the completeness of a surgical excision. However, this margin may be either nonviable/necrotic or viable. This systematic review examines the 5‐year event‐free survival rate and local recurrence as a function of positive resection margins that are nonviable/necrotic versus those that are viable. Multiple databases were searched using the Ovid interface. … Show more

“…Even though the final margin was focally positive (< 5 mm linear extent), local control can be achieved with adjuvant chemoradiation. 11,12 The tradeoff of microscopic residual tumor is a medical equipoise considering the significant morbidity of facial nerve paralysis. Indeed, in this current case, while our patient had microscopic positive surgical margin, she remains alive with no evidence of disease 2 years following Ewing sarcoma-specific neoadjuvant and adjuvant regimens.…”

“…Response to chemotherapy as determined by percent tumor cell necrosis on histopathologic evaluation is a prognostic indicator in both osteosarcoma and Ewing sarcoma 27,28 , but not pleomorphic soft-tissue sarcomas 29 . However, there are limited data on whether a margin involved by nonviable tumor, also referred to as the chemotherapy scar or tumor bed, affects LR 30,31 . In 2004, Kawaguchi et al proposed “the concept of the curative margin,” which combines margin width with the effectiveness of the preresection therapy when assessing local control in bone and soft-tissue sarcomas 32 .…”
