Viral Myocarditis: Classification, Diagnosis, and Clinical Implications

Abstract: Myocarditis is an inflammatory disease of the myocardium with focal or diffuse involvement. Viral infections are the most common cause of myocarditis, especially in Western countries. A recent viral illness with gastroenteric or upper respiratory symptoms often precedes myocarditis. The absence of specific pathognomonic features in conjunction with the wide spectrum of clinical manifestations that range from subclinical cases to sudden cardiac death (SCD) makes myocarditis diagnosis particularly challenging. “…In the current study, the occurrence of paroxysmal or sustained atrial or ventricular arrhythmias was more common in severe than in mild, cases (45.5% vs. 16.8%, p =0.008). This was also found in a previous study that reported that 25% of acute myocarditis cases exhibited cardiac arrhythmia, which was more frequently observed in severe myocarditis [ 45 , 46 ]. Furthermore, in the current study, AV nodal conduction delays or intraventricular conduction defects were more frequently observed in severe cases compared with mild cases (18.2% vs. 4.2%, p =0.034).…”

“…It sends millions of people to the hospital and causes millions of deaths. (6,7) The increased ranking of COVID as a leading cause of death in some age groups is consistent with a downward age shift in the distribution of COVID deaths in last years. (3,8) COVID and Influenza co-infection with other non-communicable chronic diseases (NCDs) complicate the diagnosis, treatment, prognosis of the ARDs emerge new concern.…”

“…In most of our patients, a patchy mid-wall and sometimes subepicardial LGE was found. This distribution pattern is typically seen during or after myocarditis ( 20 ). In COVID-19 patients with MISC or isolated heart involvement, an identical MRI image with this myocarditis type is seen in all degrees of myocardial involvement ( 21 ).…”
