Viscoelastic Testing in Pediatric Mechanical Circulatory Support

Abstract: Pediatric mechanical circulatory support can be lifesaving. However, managing anticoagulation is one of the most challenging aspects of care in patients requiring mechanical circulatory support. Effective anticoagulation is even more difficult in pediatric patients due to the smaller size of their blood vessels, increased turbulent flow, and developmental hemostasis. Recently, viscoelastic testing (VET) has been used as a qualitative measure of anticoagulation efficacy in patients receiving extracorporeal memb… Show more

“…No correlation was found between ROTEM parameters and bleeding complications (128). Clot formation time on INTEM and HEPTEM was found to have a moderate-strong correlation with aPTT and HaPTT for children receiving bivalirudin for anticoagulation (123).…”

“…In critical care settings, global hemostatic tests can guide diagnosis, management, and outcome prediction in various conditions like cardiac surgery, mechanical circulatory support, and DIC (38–40). Specifically, in pediatric DIC cases, two case reports used CWA to monitor hemostatic changes, and one case report demonstrated thromboelastometry-guided surgery in a girl with neuroblastoma and DIC (31, 32, 41).…”

“…It is important to ascertain the recent increase in the evidence to support the use of viscoelastic tests thromboelastogram (TEG)/rotatory thromboelastometry (ROTEM) to evaluate and optimise coagulopathy in the paediatric surgical population, especially, cardiac surgery (CPB), craniosynostosis surgery, patients on mechanical circulatory support and trauma 120–122 …”
