Vital pulpectomy and root filling with N2 or Endomethasone

Abstract: Summary. Using an aseptic and standardized endodontic technique, vital pulpectomy was performed on 16 pairs of human permanent maxillary front teeth scheduled for extraction in connection with prosthetic treatment. All the teeth had clinically sound pulps before treatment and 16 teeth were root filled with N2 and the other 16 with Endomethasone. In all cases except two, contralateral teeth were used. After 4–8 months the teeth were extracted and processed for histological examination using light microscopy. H… Show more

“…The irritative effects of root canal sealers have been evaluated by histopathologic examinations of the tissue response. Unfortunately, several studies have shown that most commercially available root canal sealers can induce mild to severe inflammatory alteration within apical tissues (1)(2)(3)(4). Recently, our studies have shown the activation of cyclooxygenase (COX)-2 (5-7), interleukin (IL)-6 (8), , heme oxygenase-1 (9), gelatinases (10), and tissue type plasminogen activators (10) might play an important role in induction of periapical destruction by root canal sealers.…”

“…Various histologic investigations indicate that commercially available root canal sealers tested in this study can induce apical bone destruction (1)(2)(3)(4). In this study, we first found that root canal sealers can suppress ALP mRNA gene expression and ALP activity in U2OS cells.…”

Unfortunately, several studies have shown that most commercially available root canal sealers can induce mild to severe inflammatory alteration within apical tissues, leading to bone destruction (1)(2)(3)(4). Recently, our studies have shown the activation of inflammatory mediator cyclooxygenase-2 (5-7), inflammatory cytokines interleukin-6 and interleukin-8 (8), antioxidant enzyme heme oxygenase-1 (9), proteolytic enzyme gelatinases and tissue-type plasminogen activators (10), and osteolytic marker receptor activator of nuclear factor kappa B ligand (11) might play an important role in root canal sealer-induced periapical inflammation.…”

“…With advances in etch-&-rinse and self-etching adhesive technologies [7,16], there is increasing interest in the use of low viscosity, methacrylate resin-based composites as sealers for root filling materials in endodontics [3,4,17]. The release of unreacted monomers from resin composites may stimulate the growth of bacteria [18] and promote allergic or cytotoxic reactions to intraoral tissues [18].…”
