Volumetric modulated arc therapy is superior to conventional intensity modulated radiotherapy - a comparison among prostate cancer patients treated in an Australian centre

Abstract: BackgroundRadiotherapy technology is expanding rapidly. Volumetric Modulated Arc Therapy (VMAT) technologies such as RapidArc® (RA) may be a more efficient way of delivering intensity-modulated radiotherapy-like (IM) treatments. This study is an audit of the RA experience in an Australian department with a planning and economic comparison to IM.Methods30 consecutive prostate cancer patients treated radically with RA were analyzed. Eight RA patients treated definitively were then completely re-planned with 3D c… Show more

“…This compares well with Hsu et al [22]. This treatment therefore saves machine time when compared even to other conformal techniques, as has been found in other tumour sites [15]. More importantly, VMAT minimises time spent at hospital with the single planning session and short daily fraction time.…”

“…The recent introduction of VMAT has therefore allowed the expansion of conformal techniques to more cases including palliative scenarios [15]. VMAT has enabled WBRT with hippocampal avoidance (WBRTHA), with simultaneous Integrated boost (SIB) to regions of macroscopic tumour, all within the one radiotherapy course.…”

“…In the start-up scenario, the incremental cost of imrt increased by a factor of about 11 ($4,268 rather than the $381 estimated for a mature program), which highlights the importance of evaluation timing. In most ceas, cost estimates based on a mature program are more appropriate, because it takes less than 1 year for a centre to become familiar with the technique 24 .…”

“…External beam RT (EBRT) for skin cancer has recently been revolutionised by the adoption of volumetric modulated arc therapy (VMAT), which is the newest stage in the evolution of intensity modulated RT (IMRT). 35 Tangential VMAT (TVMAT) is ideal for scalps. 17 VMAT is essentially the application of computed tomography technology to RT and can treat large fields of skin quickly and accurately with great conformality that is without overdosing surrounding dose-sensitive normal tissues.…”
