Von Willebrand Syndromes and Mitral-Valve Prolapse

Abstract: We searched for mitral-valve prolapse by two-dimensional echocardiography in 15 patients with von Willebrand syndromes to test the hypothesis that this bleeding disorder is actually a mesenchymal dysplasia that resembles the heritable disorders of connective tissue. This valvular abnormality was found in nine (60 per cent) of these patients, as compared with four (13.3 per cent) of 30 sex-matched and age-matched healthy controls. This difference was statistically significant (P less than 0.01). The association… Show more

“…This association of primary MVP with an increased incidence in patients with von Willebrand disease and other coagulopathies, primary hypomastia, and various connective tissue diseases has been used to support this concept. 39,40) Grahame, et al reported that there was a trend towards a positive correlation between anterior mitral leaflet excursion (eg, displacement of anterior mitral leaflet) and hypermobility score but this did not reach statistical significance (r = 0.23, P > 0.05). 7) Moreover, they did not study the relationship between MLD, DMR IAOS and AOD and BHS.…”

“…In searching for an explanation of this associa tion, a common autoimmune etiology was suggested as only Graves' disease was associ ated with the cardiac abnormality while thy rotoxicosis of nonimmune origin showed an incidence similar to that of the normal pop ulation. Mitral valve prolapse has been found in association with other diseases such as Marfan's syndrome [3], osteogenesis im perfecta [4], Ehlers-Danlos syndrome [5], pseudoxanthoma elasticum [6] and others [7,8],…”
