The vertebrate small intestine requires an enormous surface area to effectively absorb nutrients from food. Morphological adaptations required to establish this extensive surface include generation of an extremely long tube and convolution of the absorptive surface of the tube into villi and microvilli. In this Review, we discuss recent findings regarding the morphogenetic and molecular processes required for intestinal tube elongation and surface convolution, examine shared and unique aspects of these processes in different species, relate these processes to known human maladies that compromise absorptive function and highlight important questions for future research.

SKC se obično javlja kada je duljina tankog crijeva manja od 200 cm (67% normalne duljine), 6 a manifestira se učestalim proljevima, steatorejom, dehidracijom, elektrolitskim disbalansom te malnutricijom i gubitkom tjelesne težine. 7 Važni prognostički čimbenici pri ekstenzivnoj resekciji tankog crijeva jesu mjesto resekcije te prezervacija uzlaznoga debelog crijeva i ileocekalne valvule.…”

SAŽETAK. Sindrom kratkog crijeva očituje se malapsorpcijom koja je posljedica opsežne resekcije crijeva. Duljina crijeva preostaloga nakon kirurške resekcije smatra se glavnom odrednicom ishoda u tih bolesnika. Liječenje sindroma složeno je i nalaže multidisciplinarni pristup radi smanjenja morbiditeta i mortaliteta. Ovaj prikaz opisuje 60-godišnju bolesnicu koja je podvrgnuta multiplim resekcijama crijeva, što je rezultiralo preostalim tankim crijevom duljine 30 cm postduodenalno i terminalnom jejunostomom. Njezin boravak u jedinici intenzivnog liječenja zakomplicirao se zbog razvoja respiratornog zatajenja, bilateralne pneumonije i sepse. Pacijentica je mehanički ventilirana i liječena antimikrobnim lijekovima u skladu s rezultatima mikrobioloških pretraga i antibiogramom. Inicijalno je započeta potpuna parenteralna prehrana, dok se enteralna prehrana uvodila postupno. U liječenju bolesnice sudjelovao je multidisciplinarni tim sastavljen od anesteziologa, kirurga i gastroenterologa. Nakon četiri mjeseca bolničkog liječenja pacijentica je otpuštena kući opremljena tuneliranim središnjim venskim kateterom, a parenteralna je prehrana nastavljena u kućnim uvjetima u kombinaciji s peroralnim hranjenjem.

Diarrhea after bariatric procedures, mainly those with malabsorptive elements including Roux-Y Gastric Bypass and Biliopancreatic Diversion, is common and an essential determinant of quality of life and micro- and macronutrient deficiencies. Bariatric surgery is the only sustainably successful method to address morbid obesity and its comorbidities, particularly gaining more and more importance in the specific treatment of diabetic patients. Approximately half a million procedures are annually performed around the world, with numbers expected to rise drastically in the near future. A multitude of factors exert their influence on bowel habits; preoperative comorbidities and procedure-related aspects are intertwined with postoperative nutritional habits. Diagnosis may be challenging owing to the characteristics of post-bariatric surgery anatomy with hindered accessibility of excluded segments of the small bowel and restriction at the gastric level. Conventional testing measures, if available, generally yield low accuracy and are usually not validated in this specific population. Limited trials of empiric treatment are a practical alternative and oftentimes an indispensable part of the diagnostic process. This review provides an overview of causes for chronic post-bariatric surgery diarrhea and details the particularities of its diagnosis and treatment in this specific patient population. Topics of current interest such as the impact of gut microbiota and the influence of bile acids on morbid obesity and especially their role in diarrhea are highlighted in order to provide a better understanding of the specific problems and chances of future treatment in post-bariatric surgery patients.
