D iffusion-weighted imaging (DWI) measures the degree of water mobility, i.e., random Brownian motion, in vivo and is a noninvasive tool (1-3). DWI has been used mainly in cranial magnetic resonance imaging (MRI) applications to visualize stroke, neoplasms, intracranial infections, traumatic brain injury, and demyelinating processes since early 1990s (4-8). However, in recent years, DWI applications has been extended to breast, musculoskeletal, liver, prostate, pelvis, and general whole body imaging with the development of multichannel coils, parallel imaging, faster gradients, and MRI hardware (9)(10)(11)(12)(13)(14). DWI can provide a quantitative map of water diffusion coefficient. Water diffusion coefficient can be calculated from diffusion-weighted images using at least two different DWI values. DWI is achieved by applying diffusion gradients and is called the b-value. Water diffusion coefficient in the tissue is called apparent diffusion coefficient (ADC) and can be calculated from diffusion-weighted images using a linear regression analysis. The term "apparent" is used for diffusion coefficient to differentiate from true diffusion coefficient since the measured water diffusion coefficient in the tissue is influenced by a number of other factors such as capillary network orientation and gross motion in addition to random Brownian motion. ADC measurements are considered to be of greater importance in differential diagnosis of various pathological conditions and its accurate measurement is of great importance (12,13,(15)(16)(17)(18).In the past, the magnetic resonance gradients were much slower and repetition time (TR) and echo-time (TE) were quite long. Thanks to the fast pace of advancement in MRI, the imaging parameters were shortened significantly. Therefore, TR and TE could be reduced in such a way that they could be comparable to tissue relaxation times (T1 and T2) in order to reduce susceptibility artifacts and the total scan time for various DWI applications. PURPOSEWe aimed to investigate the effect of key imaging parameters on the accuracy of apparent diffusion coefficient (ADC) maps using a phantom model combined with ADC calculation simulation and propose strategies to improve the accuracy of ADC quantification. METHODSDiffusion-weighted imaging (DWI) sequences were acquired on a phantom model using single-shot echo-planar imaging DWI at 1.5 T scanner by varying key imaging parameters including number of averages (NEX), repetition time (TR), echo time (TE), and diffusion preparation pulses. DWI signal simulations were performed for varying TR and TE. RESULTSMagnetic resonance diffusion signal and ADC maps were dependent on TR and TE imaging parameters as well as number of diffusion preparation pulses, but not on the NEX. Apparent Diffusion Coefficient Values of the Benign Central Zone of the Prostate: Comparison With Low- and High-Grade Prostate Cancer

Objective The apparent diffusion Coefficient (ADC) values for benign central zone (CZ) of the prostate were compared with ADC values of benign periphral zone (PZ), benign transition zone (TZ), and prostate cancer, using histopathologic findings from radical prostatectomy as the reference standard. Materials and Methods The study included 27 patients with prostate cancer (mean [± SD] age, 60.0 ± 7.6 years) who had 3-T endorectal coil MRI of the prostate performed before undergoing prostatectomy with whole-mount histopathologic assessment. Mean ADC values were recorded from the ROI within the index tumor and within benign CZ, PZ, and TZ, with the use of histopathologic findings as the reference standard. ADC values of the groups were compared using paired t tests and ROC curve analysis. Results The ADC of benign CZ in the right (1138 ± 123 × 10−6 mm2/s) and left (1166 ± 141 × 10−6 mm2/s) lobes was not significantly different (p = 0.217). However, the ADC of benign CZ (1154 ± 129 × 10−6 mm2/s) was significantly lower (p < 0.001) than the ADCs of benign PZ (1579 ± 197 × 10−6 mm2/s) and benign TZ (1429 ± 180 × 10−6 mm2/s). Although the ADC of index tumors (1042 ± 134 × 10−6 mm2/s) was significantly lower (p = 0.002) than the ADC of benign CZ there was no significant difference (p = 0.225) between benign CZ and tumors with a Gleason score of 6 (1119 ± 87 × 10−6 mm2/s). In 22.2% of patients (6/27), including five patients who had tumors with a Gleason score greater than 6, the ADC was lower in benign CZ than in the index tumor. The AUC of ADC for the differentiation of benign CZ from index tumors was 72.4% (sensitivity, 70.4%; specificity, 51.9%), and the AUC of ADC for differentiation from tumors with a Gleason score greater than 6 was 76.7% (sensitivity, 75.0%; specificity, 65.0%). Conclusion The ADC of benign CZ is lower than the ADC of other zones of the prostate and overlaps with the ADC of prostate cancer tissue, including high-grade tumors. Awareness of this potential diagnostic pitfall is important to avoid misinterpreting the normal CZ as suspicious for tumor.

Abstract: • Multiple b ≥ 1,000 s/mm ( 2 ) 3 T-DW Magnetic Resonance Imaging provides excellent prostate cancer depiction. • Prostate DWI and ADC maps are attainable at 3 T without endorectal coil. • Prostate cancer depiction is improved on high resolution b 1,500 s/mm ( 2 ) 3 T-DWI.
