Coronavirus disease 2019 (COVID-19) has been reported as the possible cause of acute myocarditis. Myocarditis is an inflammatory heart disease mostly caused by viral infections. Cytomegalovirus (CMV) primary infection is often not suspected as a cause of myocarditis in immune-competent adults. We report the case of a 37-year-old male admitted with fever, cough and dyspnea. Chest CT showed typical ground-glass changes indicative of viral pneumonia. He was tested negative for COVID-19 but had biological markers that made us still suspect it. He had elevated troponin I level (up to 111.5 ng/mL) and diffuse myocardial dyskinesia along with a decreased left ventricular ejection fraction (LVEF). He was diagnosed with CMV myocarditis with cardiac insufficiency and totally recovered without antiviral therapy. During the COVID-19 pandemic patients may develop myocarditis, still every myocarditis is not a COVID infection. Myocarditis linked to CMV infection may be rare, but life-threatening.

While COVID-19 initially appeared to be manifested primarily by respiratory symptoms, it is now known to be a multi-organ disease, also with dermatological manifestations. Cutaneous symptoms in patients with COVID-19 have a wide clinical spectrum. They may appear at any stage of the disease or as its sole manifestation. Further research is necessary to clarify the pathogenesis of these changes, their prognostic significance and therapeutic management. This article summarizes the current knowledge about changes occurring in the skin, mucous membranes and nails of patients diagnosed with COVID-19 and addresses the issue of drug-induced skin changes associated most common treatments of COVID-19. StReSzCzeNIe Mimo że początkowo wydawało się, że COVID-19 manifestuje się głównie objawami ze strony układu oddechowego, obecnie wiadomo, że jest to schorzenie wielonarządowe, a symptomy obejmują także skórę. Objawy skórne u pacjentów z COVID-19 wykazują szerokie spektrum kliniczne i mogą pojawić się na każdym etapie choroby, jak również być jedyną jej manifestacją. Niezbędne są dalsze badania, które pozwolą na wyjaśnienie patogenezy tych zmian, ich znaczenia prognostycznego oraz ustalenie postępowania terapeutycznego. W pracy podsumowano aktualny stan wiedzy na temat zmian w obrębie skóry, błon śluzowych oraz aparatu paznokciowego obserwowanych u pacjentów z rozpoznaniem choroby, a także wymieniono możliwe skórne działania niepożądane leków najczęściej stosowanych u tej grupy chorych.
