?Z? flow hydrocephalus shunt, a new approach to the problem of hydrocephalus, the rationale behind its design and the initial results of pressure monitoring after ?Z? flow shunt implantation

Abstract: The over-drainage of CSF after shunt implantation, particularly when the patient is in the vertical posture, is well documented. Problems associated with it are negative pressure syndrome (postural headache and postural irritability), abnormal skull size, craniosynostosis and subdural haematoma. The pressure gradient in the vertical position between the cranial cavity and the right atrium is 15 to 20 cm of water and between the cranial and peritoneal cavity it is 25 to 32 cm of water and both these are much hi… Show more

“…Advanced understanding was accompanied by new concepts in the management of hydrocephalus [4,9,12,14] and by improvement in shunt technology [5,11,22,23,25]. Based on his interpretations of the pathophysiology of hydrocephalus S. Hakim searched for a valve adaptable to changing hydrodynamic conditions [11].…”

“…The problem of overdrainage of differential pressure valves created new concepts such as the anti-siphon device or the flow-regulated shunt system [5,[22][23][24]. But not all experiences have been as encouraging as the underlying theoretical considerations and some problems still remain unsolved [1,2,15,19,30].…”

“…Several studies have confirmed their efficacy and functionality on a par with the standard shunt systems. [29] They have “Slit N Spring”, “Z” flow, low-pressure, medium-pressure, and high-pressure valves. These valves were evaluated and compared with the western systems and found to be equally effective.…”

“…18 Craniosynostosis can also be found when patients are overshunted and sutures subsequently override and fuse. 19,20 The condition may be "isolated," involving a single suture, or "complex," involving multiple sutures. 17,21 Approximately 100 different forms have been described.…”
