Zoledronic acid to prevent bone loss in the first 6 months after renal transplantation

Abstract: Our results show that administration of zoledronic acid improves the calcium content of cancellous bone after kidney transplantation. The beneficial effect of bisphosphonate therapy is further evidenced by an increase of lumbar spine BMD, and stabilization of femur BMD.

“…Nonetheless, we were able to distinguish among the various classifications of renal osteodystrophy on the basis of the presence and the quality of the cellular and architectural elements. This technique has been useful to classify renal osteodystrophy (22,29,30). The cellular parameters of bone formation and resorption (osteoclasts and osteoblasts number/bone length) revealed an impressive decline after 6 mo of pamidronate treatment.…”

“…When concerning with bone formation markers in post-transplant osteoporosis, controversial results were reported in serum osteocalcin levels during bisphosphonate treatment. 43,44 Side effects of the treatments employed were generally mild and well controlled by appropriate premedications. No alteration in kidney function was detected at any time point during the study, and liver enzymes did not worsen during the treatment period.…”

“…Trials using zoledronic acid has also given positive results 55. Calcitonin seems to be ineffective in preventing early bone loss, but may have some benefit in the later post-transplant period in liver56 and renal57 transplant recipients and can be considered a safe alternative if other agents are contraindicated or poorly tolerated.…”
