Randomized trials have provided stronger scientific evidence regarding the effectiveness of screening for breast cancer than for any other cancer. However, much still needs to be learned. Periodic gatherings of scientists in the field should speed the process.
Incidence data are presented for the 7 most frequent specific categories of mental disorder available in the NIMH Epidemiologic Catchment Area (ECA) program (major depressive disorder; panic disorder; phobic disorder; obsessive-compulsive disorder; drug abuse/dependence; alcohol abuse/dependence; cognitive impairment). The DSM-III case definitions in the ECA Program are according to the implementation of the Diagnostic Interview Schedule (DIS). Rates of incidence are presented specific for age, sex, and site, and pooled smoothed curves for the relationship of age to incidence, specific for sex are shown. The 7 disorders have distinctly different relationships to sex and age of onset.
Three health education interventions for urban poor hypertensive patients were introduced sequentially in a randomized factorial design: 1) an exit interview to increase understanding of and compliance with the prescribed regimen; 2) a home visit to encourage a family member to provide support for the patient's regimen; and 3) invitations to small group sessions to increase the patient's confidence and ability to manage his/her problem. Previous evaluation of the initial two-year experience demonstrated a positive effect of the educational program on compliance with the medical treatment and blood pressure control. A 30-year follow-up of 1,758 inner-city children and their mothers in the Pathways to Adulthood Study revealed significant associations in transgenerational timing of age at 1st birth between mothers and their daughters and sons. Intergenerational age patterns were associated with the children's family and personal characteristics during childhood and adolescence and self-sufficiency at age 27-33. Continuity in teenage parenthood was associated with family and personal characteristics unfavorable for optimal child development and successful adult outcomes. Delay in 1st parenthood to age 25 or older was associated with significantly greater odds of more favorable environmental and developmental characteristics and greater adult self-sufficiency. The authors concluded that age at 1st birth of both mothers and children contributes, but in subtly different ways for daughters and sons, to the children's development and adult self-sufficiency.
