Purpose To prospectively develop individualized low-volume contrast media (CM) protocols adapted to tube voltage in patients undergoing computed tomographic (CT) angiography of the aorta. Materials and Methods The study was approved by the institutional review board and local ethics committee. All patients provided written informed consent. CT angiography was performed by using automated attenuation-based tube voltage selection (ATVS) (range, 70-150 kVp; 10-kVp increments). Iodine attenuation curves from an ex vivo experiment in a phantom were used to design CM protocols for CT angiography of the thoracoabdominal aorta in 129 consecutive patients (hereafter, cohort A). Further modified CM protocols based on results in cohort A were designed with the aim of homogeneous vascular attenuation of 300-350 HU across tube voltages and were applied to another 61 consecutive patients (cohort B). Three independent blinded radiologists assessed subjective image quality, and one reader determined objective image quality. The Kruskal-Wallis test was performed to test for differences in subjective image quality, and linear regression was performed to test for differences in objective image quality between the automatically selected tube voltages. Results Experiments revealed tube voltage-dependent iodine attenuation curves, which were used to determine the CM protocols in cohort A; these ranged from 68 mL at 110 kVp to 45 mL at 80 kVp. In both cohorts, ATVS selected 80 kVp in 62 patients, 90 kVp in 84, 100 kVp in 33, and 110 kVp in 11. In cohort A, image quality that was satisfactory or better was attained in 126 (98%) of 129 patients who had no significant differences in subjective image quality between tube voltages (P = .106) but who did have significant differences in attenuation and contrast-to-noise ratio (CNR) (P < .001 for both). In cohort B, the further-modified CM protocol (from 33 mL at 80 kVp to 68 mL at 110 kVp) yielded image quality that was satisfactory or better in all 61 (100%) patients, without significant differences in subjective image quality (P = .178), and without significant differences between tube voltage and attenuation (P = .108), noise (P = .250), or CNR (P = .698). Conclusion Individualized low-volume CM protocols based on automatically selected tube voltages are feasible and yield diagnostic image quality for CT angiography of the aorta. C omputed tomography angiography (CTA) of the aorta is the reference method to investigate aortic disease and to follow up patients after surgical or endovascular aortic repair, either thoracic or abdominal. Thoracic aorta usually demands electrocardiography (ECG)-gated CTA to avoid motion artifacts related to cardiac cycle, notably for imaging aortic root and ascending segment (1). However, ECG-gating results in significantly increased radiation exposure, notably in case of low-pitch acquisition for retrospective reconstruction. Furthermore, ECG-gating technique is also related to significant increase of imaging time and volume of contrast agent.Dual-source (DS) hardware is a significant technological advance because it allows acquisition with exceptionally high-pitch and fast rotation speed, with substantial reduction of acquisition time and time resolution. For instance, DS-CTA with ECG-gating allows whole coronary imaging (scan length, 12-14 cm) in a single heart cycle with significant reduction in radiation dose and volume of contrast agent (2). Furthermore, DS-CTA with ECG-gating provides high quality images with minor motion artifacts also in imaging of whole thoracic aorta (scan length 20-30 cm) (3). Karlo et al. (4) demonstrated that high-pitch DS-CTA without ECG-gating provides diagnostic image quality of the aortic valve-aortic root complex similar to ECG-gated technique. Despite these advantages in acquisition of short vascular segments (e.g., coronary or thoracic aorta), the role of high-pitch DS-CTA in imaging the whole thoracoabdominal aorta still remains a challenge and a topic of debate (5-8) C A R D I O VA S C U L A R I MAG I N G O R I G I N A L A R T I C L E PURPOSEWe aimed to perform intraindividual comparison of computed tomography (CT) parameters, image quality, and radiation exposure between standard CT angiography (CTA) and high-pitch dual source (DS)-CTA, in subjects undergoing serial CTA of thoracoabdominal aorta. METHODSEighteen subjects with thoracoabdominal CTA by standard technique and high-pitch DS-CTA technique within 6 months of each other were retrieved for intraindividual comparison of image quality in thoracic and abdominal aorta. Quantitative analysis was performed by comparison of mean aortic attenuation, noise, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR). Qualitative analysis was performed by visual assessment of motion artifacts and diagnostic confidence. Radiation exposure was quantified by effective dose. Image quality was apportioned to radiation exposure by means of figure of merit. RESULTSMean aortic attenuation and noise were higher in high-pitch DS-CTA of thoracoabdominal aorta, whereas SNR and CNR were similar in thoracic aorta and significantly lower in high-pitch DS-CTA of abdominal aorta (P = 0.024 and P = 0.016). BackgroundThe aim of this study was to investigate the diagnostic value of multi-slice spiral computed tomography (MSCT) for bronchial dysplasia in premature infants.Material/MethodsA retrospective analysis of 248 premature infants who were highly suspected to have bronchial dysplasia and were admitted to our hospital from 2015 onwards was conducted. We observed bronchus morphologies, sizes, and tissue characteristics using fiberoptic bronchoscopy (FB) as the criterion standard for diagnosis. We calculated the sensitivity, specificity, and diagnostic compliance of MSCT in the diagnosis of bronchial dysplasia.ResultsThoracic computed tomography mainly revealed capsular bubbles. The translucency of the 2 lungs was reduced, and extensive and local ground-glass changes were observed. Imaging findings mostly included strip or honeycomb-like shadows. Pleural thickening and pleural effusion were rare. MSCT was able to establish a diagnosis in 92 cases (37.10%) of bronchopulmonary cysts, 69 cases (27.82%) of congenital pulmonary emphysema, 31 cases (12.50%) of bronchial atresia, 1 case (0.40%) of congenital cystadenoma malformation, and 3 cases (1.21%) of giant tracheal bronchitis. Another 52 children (20.97%) were found to have conventional pulmonary inflammation. The sensitivity of MSCT in the diagnosis of bronchial dysplasia was 88.21%, the specificity was 75.00%, and the diagnostic compliance was 86.29%. There was a significant difference between the MSCT and FB findings in the diagnosis of bronchial hypoplasia (P<0.001).ConclusionsMSCT has great utility in the diagnosis of bronchial dysplasia in premature infants and may become an excellent method for diagnosing bronchial dysplasia in the future.
