18F-FDG-PET/CT in SARS-CoV-2 infection and its sequelae

Abstract: In recent months, much of the scientific efforts have focused on research on SARSCoV-2 infection and its consequences in humans. Still, many aspects remain unknown. It is known that the damage caused by SARS-CoV-2 is multifactorial and that its extension goes beyond lung inflammation and the acute phase, with the appearance of numerous complications and sequelae. To date, knowledge about the usefulness of 18 F-FDG-PET/CT in the acute phase has been limited to the incidental detection of … Show more

“…Indeed, many survivors of COVID-19 have chronic post-viral complications similar to the previous severe acute respiratory syndrome (SARS) and Middle East respiratory syndrome pandemics [6]. A generally accepted standardized clinical case definition of this post-viral state is lacking [7]. Various terminologies and definitions, including long COVID, COVID long-haulers, post-acute COVID-19, late sequelae of COVID-19, or post-COVID-19 have been proposed and controversies still exist about the correct naming.…”

“…18 F-FDG-PET/CT can be a useful tool to detect or rule out severe concomitant processes. Alterations of brain metabolism shown with this technique may be a marker of the systemic process [7]. However, a review of literature focused on amalgamating the results of PET scans in post-COVID-19 patients has not been undertaken.…”

“…SARS-CoV-2 virus mainly caused viral pneumonia with intense F-18 FDG uptake during the infectious process. However, persistent F-18 FDG uptake in the lung parenchyma could be observed even the improvement of clinical symptoms and CT finding, while F-18 FDG uptake remained unknown on the exact time period for resolution [ 26 , 27 ]. Persistent intense F-18 FDG uptake in lung parenchyma even at the recovery stage might potentially indicate a high level of inflammatory process for prediction of further damages to lung parenchyma [ 27 ].…”

“…The urgency of imaging patients acutely ill with COVID-19 as well as following up post-COVID patients and those receiving vaccines, did not allow for time to create SARS-CoV-2 specific imaging agents. The domineering modality so far has been FDG-PET/CT covered in several excellent reviews (55,56) while SPECT imaging also contributes, for example, toward imaging of pulmonary emboli in COVID patients (57). As COVID-19 will gradually become endemic, it is only a matter of time when COVID-specific PET and SPECT imaging agents are developed.…”
