Thirty-eight female patients (age range: 31-79 years) with breast masses underwent technetium-99m methoxyisobutylisonitrile (99mTc-MIBI) breast scintigraphy in order to assess the value of this technique in the detection of breast carcinomas and in their differentiation from benign breast masses. The results showed that 27 of the 32 cases of breast carcinoma were detected by 99mTc-MIBI breast scintigraphy. In contrast, none of the six benign lesions could be detected by this method. The diagnostic sensitivity, specificity and accuracy were 84%, 100% and 87%, respectively, in the differentiation of malignant and benign breast masses. In conclusion, we consider 99mTc-MIBI breast scintigraphy is useful in distinguishing malignancies from benign breast masses.

In a prospective study we examined 38 patients with primary bronchogenic carcinoma to validate the use of indium-111 pentetreotide (IPT) as a diagnostic tool. Of these 38 patients, 25 had small cell lung cancer (SCLC) and 13, non-small cell lung cancer (NSCLC). The aim of the study was to investigate whether (a) the disease can be reliably detected, (b) IPT allows differentiation between SCLC and NSCLC and (c) IPT provides further information on metastatic disease. After giving their informed consent the patients were injected and imaged 4 and 24 h later using a planar whole-body technique. In addition single-photon emission tomography of the thorax and, if necessary, other areas of the body was performed at 24 h. In the 25 patients with SCLC 22 sites of primary tumour were correctly identified (true-positive, TP); one was false-negative (FN) and two were true-negative (TN), the patients being in full remission. Metastases were correctly identified in ten instances (lung, bone and brain), while the findings were FN in five cases. An additional six FN findings resulted in the area of the upper abdomen due to the physiological uptake in the liver, spleen and kidneys. In the 13 patients with NSCLC, ten findings were TP and 3 FN with respect to the primary tumour. Two FNs were squamous cell carcinoma, and one, adenocarcinoma. Metastases were TP in nine cases and FN in one.(ABSTRACT TRUNCATED AT 250 WORDS)

Contribuição da medicina nuclear para a avaliação dos linfomas A medicina nuclear permite a caracterização funcional de tecidos, acrescentando dados à avaliação anatômica realizada por outros métodos de imagem, como a tomografia computadorizada, ressonância magnética ou ultrasom. Além do diagnóstico e estadiamento, as informações funcionais fornecidas pela medicina nuclear são particularmente úteis no seguimento e avaliação de resposta terapêutica dos pacientes com linfoma. A presença de massa residual após quimioterapia ou a infiltração de linfonodos de pequenas dimensões são exemplos de situações nas quais a avaliação isolada de características anatômicas é insuficiente. Os principais métodos utilizados são a cintilografia com gálio-67, traçador com alta afinidade por transferrina e receptores de ferro, e os estudos com flúor-deoxiglicose, emissor de pósitron que permite a caracterização do metabolismo de glicose nos tecidos. Outros traçadores, como o tálio-201, sestamibi-Tc99m e octreotide-In111, também são utilizados em menor escala na avaliação dos pacientes com linfoma. Os mecanismos de captação, principais indicações dos estudos, limitações e resultados de literatura serão revistos.
