31-Year-Old South African Man With Fever and Headache

Abstract: A 31-year-old man with no medical history presented to the emergency department (ED) with a 1-week history of worsening headaches, myalgia, and high-grade fever. He had arrived in Minnesota from South Africa a few days before onset of symptoms to work on a local farm. In South Africa, he worked near Durban as a farmer tending sheep and cattle in open fields.
