IntroductionModifiable risk factors account for a substantial proportion of Alzheimer’s disease (AD) cases and we currently have a discrete AT(N) biomarker profile for AD biomarkers: amyloid (A), p-tau (T), and neurodegeneration (N). Here, we investigated how modifiable risk factors relate to the three hallmark AT(N) biomarkers of AD.MethodsParticipants from the European Prevention of Alzheimer’s Dementia (EPAD) study underwent clinical assessments, brain magnetic resonance imaging, and cerebrospinal fluid collection and analysis. Generalized additive models (GAMs) with penalized regression splines were modeled in the AD Workbench on the NTKApp.ResultsA total of 1,434 participants were included (56% women, 39% APOE ε4+) with an average age of 65.5 (± 7.2) years. We found that modifiable risk factors of less education (t = 3.9, p < 0.001), less exercise (t = 2.1, p = 0.034), traumatic brain injury (t = −2.1, p = 0.036), and higher body mass index (t = −4.5, p < 0.001) were all significantly associated with higher AD biomarker burden.DiscussionThis cross-sectional study provides further support for modifiable risk factors displaying neuroprotective associations with the characteristic AT(N) biomarkers of AD.

The World Health Organization forecasts a population of 2,000 million people over 60 years by the year 2050, with 7% of this population suffering from dementia, a disease of public priority. Making a constant evaluation of older adults allows early detection of the disease and provides a better quality of life in the patient. In this sense, the research and development of innovative technological systems for the management of the growing number of patients with cognitive diseases has increased in recent years, integrating data collection and its automatic processing based on geriatric metrics into these systems using artificial intelligence (AI) methods, such that they can establish disease detection at an early stage and follow-up of it, in order to support the increase in patients expected in the coming years in the clinical area. This research presents an interactive web platform that allows users with internet connection from any mobile device, computer, laptop, or other devices, to remotely perform an automated assessment of the Montreal Cognitive Assessment (MoCA) test. This test detects and assesses cognitive deterioration. We use AI and neural network methods for binary and multiclass classification to obtain assessment scores according to geriatric metrics. Subsequently, this test is validated remotely by a mental health specialist. The tests carried out show a correct correspondence in the handling of the information and the results regarding the reference data for comparison. Our system provides an automated and easyto-use digital evaluation metric.

Background: early detection of dementia and Mild Cognitive Impairment (MCI) have an utmost significance nowadays, and smart conversational agents are becoming more and more capable. DigiMoCA, an Alexa-based voice application for the screening of MCI, was developed and tested. Objective: to evaluate the acceptability and usability of DigiMoCA, considering the perception of end-users and cognitive assessment administrators, through standard evaluation questionnaires. Method: a sample of 46 individuals and 24 evaluators participated in this study. End-users were fairly heterogeneous considering demographic and neuro-psychological characteristics. Evaluators were mostly health and social care professionals, relatively well-balanced in terms of gender, career background and years of experience. Results: end-users acceptability ratings were generally positive (rating above 3 in a 5-point scale for all dimensions) and it improved significantly after the interaction with DigiMoCA. Administrators also rated the usability of DigiMoCA, with an average score of 5.86/7 and with high internal consistency ($$\alpha $$ α = 0.95). Conclusion: although there is still room for improvement in terms of user satisfaction and voice interface, DigiMoCA is perceived as an acceptable, accessible and usable cognitive screening tool, both by individuals being tested and test administrators.
