5-Year Follow-Up of Polytetrafluoroethylene-Covered Stents Compared With Bare-Metal Stents in Aortocoronary Saphenous Vein Grafts

Abstract: The long-term prognosis for diseased SVGs requiring PCI is dismal. The JOSTENT PTFE stent-graft results in inferior outcomes compared with BMS, despite high-pressure implantation and prolonged dual antiplatelet therapy, a finding that becomes more evident with longer-term follow-up.

“…In addition, long-term patency of PTFE-covered coronary stents might be inferior as restenosis rates of up to 21% 10 months after treatment of coronary artery aneurysms have been reported 8. In the treatment of saphenous vein graft stenosis, the use of PTFE-covered stents compared with bare metal stents led to a higher rate of restenosis (68.3 vs. 51.8%, p  = 0.007) after 5 years 9. Although the surgical approach is more invasive, the complete removal of the hematoma was achievable and relief of hematoma-induced compression of the distal coronary artery was provided.…”

“…The third consideration is the restenosis issue. In one study, after coronary stenting restenosis occurred in 25% of patients17). A covered stent, such as, the Jostent Graftmaster, is a stainless steel stent with the PTFE membrane placed between 2 layers of stent struts.…”

“…Although conduits in combination with the internal mammary artery have partly replaced the use of saphenous veins during CABG procedures, graft patency rates remain overall unsatisfactory (24). Moreover, shortand long-term outcomes of PCI in saphenous vein graft are poor with drug-eluting stents as well as bare-metal stents (25,26). Therefore, the need to revascularize native coronary CTO with prior CABG is high.…”
