648. BioFire® FilmArray® Pneumonia plus Panel Performance Evaluation: A Multicenter, International Collaborative Study

Abstract: Background Classical methods to identify causes of community acquired, healthcare and ventilator associated pneumonia can be insensitive and slow, leading to unnecessary or inappropriate antimicrobial therapy. The BioFire® FilmArray® Pneumonia plus Panel (PNplus) detects 15 bacteria (in semi-quantitative log bin values from 10^4 to > 10^7), 7 antibiotic resistance markers (mecA/C/MREJ, CTX-M, KPC, VIM, IMP, NDM, OXA-48 like), 3 atypical bacteria (AB), and 8 viral classes directly from … Show more

“…A recent study evaluating the diagnostic yield of the panel versus a pneumonia multi-test bundle determined the panel was twice as likely to yield determination of the bacterial etiology for community-acquired pneumonia (CAP) while costing $60 less per case for testing [Gilbert et al, 2020] . Moreover, in a multinational evaluation among 52 laboratories, the BioFire PN plus was associated with a 20% increase in detections of one of more pathogens per specimen compared to standard of care testing [Ginocchio et al, 2020] . The increase in diagnostic yield is likely multifactorial.…”

“…However, the BioFire PN/PN plus may assist in this adjudication of infection vs colonization through semi-quantitative results. Several studies have examined the utility of using the panel's semi-quantitative values to assist in the differentiation of bacterial colonization versus infection [ Ginocchio et al, 2020 , Buchan et al, 2020 , Lee et al, 2019 , Yoo et al, 2020 , Edin et al, 2020 ]. Interpretation can be variable depending on several factors, including patient population, type of pneumonia (e.g.…”
