“…Despite the improvement of imaging examinations and intracystic fluid analysis, the diagnosis of SCA often remains uncertain, and lesions larger than 2 cm are frequently surgically resected in asymptomatic patients [3]. In a large retrospective multinational study on 2622 patients with SCA, 36 % of them underwent surgery for an uncertain diagnosis [11]. This approach remains inappropriate given the fact that surgery is associated with substantial morbidity and mortality [12] and that simple surveillance could be sufficient [13].…”

“…Jaundice, due to bile duct compression, is infrequent [18]. A recently published multinational, retrospective study including 598 patients with a diagnosis of SCA showed that nonspecific abdominal pain was reported in 29% of cases, diabetes mellitus in 7% of patients and, finally, bilio-pancreatic symptoms, including typical pancreatic pain, acute pancreatitis, jaundice and steatorrhoea in 7% of cases [19]. Tumor rupture, hemoperitoneum and acute surgical abdomen have also been described [20,21].…”

“…Indeed, SCA-related mortality is nil, whereas operative mortality is not. A multinational, retrospective study involving 58 centers in 18 countries showed that the post-operative mortality reported in patients that underwent pancreatic surgery for SCA was 0.8%, while the SCA related mortality was 0% in patients with a median follow-up period of 3.1 years [19]. Therefore, this latter strategy is highly recommended, and surgery should only be considered in a subset of patients with clear indications.…”

“…4 Those with no, or very low malignant potential, such as pseudocysts and SCAs, require minimal or no follow up in the absence of symptoms related to the cyst. 5 Solid-pseudopapillary neoplasms (SPN) are low-grade malignant neoplasms, and surgically resection is recommended. 6 Invasive adenocarcinoma occurs in between 4% to 16% of surgically resected MCNs in modern studies.…”
