99mTc-MIBI SPECT/CT and FDG PET/CT in Isolated Bilateral Renal Metastases From Adenoid Cystic Carcinoma of the Maxilla

Abstract: Renal metastasis from adenoid cystic carcinoma of the head and neck is uncommon. We present 99m Tc-MIBI SPECT/CT and FDG PET/CT findings in a case with isolated bilateral renal metastases from adenoid cystic carcinoma of the left maxilla. The metastatic adenoid cystic carcinomas of the kidneys showed photopenia on 99m Tc-MIBI SPECT/CT and increased FDG uptake on FDG PET/CT mimicking primary renal cell carcinoma.

“…Even more PET/CT imaging using 68 Ga-labelled FAP inhibitors ( 68 Ga-FAPI-PET/CT) detected additional metastases, thereby increasing the accuracy of staging and radiotherapy planning volumes relative to conventional CT/MRI [ 24 ]. In addition, 99mTc-MIBI SPECT/CT and FDG PET/CT showed promising results in the detection of distant metastases in ACC of the head and neck [ 25 ].…”

“…The renal cell carcinomas typically show photopenia on 99m Tc-MIBI SPECT/CT, whereas renal oncocytomas and lipid-poor angiomyolipomas tend to show increased 99m Tc-MIBI uptake. [5][6][7][8][9]11 This case indicates juxtaglomerular cell tumor should be included in the differential diagnosis of 99m Tc-MIBI-photopenic renal solid tumors including renal cell carcinoma, 5 metastasis, 10 and metanephric adenoma. 19…”
