A 2-year study of lipid and lipoprotein changes in postmenopausal women with tibolone and estrogen-progestin

Abstract: Some changes accompanying tibolone may be favorable and may counter its adverse effect on HDL cholesterol. Lowering of lipoprotein (a) was the only significant change found with estrogen-progestin.

“…However, they also consistently reduce plasma total cholesterol and lipoprotein(a). In addition, tibolone reduces plasma triglycerides, whereas continuous combined EJNETA reduces plasma LDL‐C, all changes which protect against cardiovascular disease 23–27 . Since the relative contribution of alterations in individual lipid fractions on the risk of cardiovascular disease is unknown and since the alterations in lipid metabolism associated with HRT contribute 30% to the reduction of cardiovascular diseas 28 , the overall effect of these two preparations cannot exactly be determined.…”

“…Its main advantage over estrogen therapy appears to be the lower incidence of vaginal bleeding and breast tenderness 116 . The major disadvantages of tibilone therapy compared with that of estrogen therapy is its less favorable effects on serum lipid profile, which may result in decreased cardioprotective effects 117,118 . Long‐term tibilone therapy is not associated with a decrease in LDL cholesterol as occurs with estrogen therapy, and it maintains or reduces the pretreatment levels of HDL cholesterol (whereas estrogen therapy increases it) 52 , 119 .…”
