A 34‐year‐old asymptomatic gossypiboma: A fortuitous diagnosis revealed by appendicular peritonitis: A case report

Abstract: Gossypiboma is a retained surgical sponge. Patients are usually symptomatic leading to early removal of this entity. We are reporting a 33‐year‐old asymptomatic gossypiboma discovered by appendicular peritonitis on the radiological images. The 64‐year‐old patient underwent a laparotomy with excision of the gossypiboma. The postoperative period was uneventful.

“…The delayed response involves formation of adhesions, granulomas or pseudo tumors. The patient may present with nonhealing wounds or discharging sinuses, or develop secondary complications like intestinal obstruction and fistula formation [11]. Their radiological features may simulate a hydatid cyst or tumors such as retroperitoneal tumors, desmoid tumors, ovarian teratoma, mesenchymal tumors and pancreatic tumors [12].…”

“…Gossypibomas have a variable onset and presentation. Gossypibomas have been reported most commonly after abdominal and gynecological surgeries [1]. The timeline of symptom presentation can vary from weeks to months after surgery.…”

“…If there is an exudative reaction, the patient may present with an acute abscess formation. Alternatively, a granulomatous reaction may cause asymptomatic abdominal adhesions and aseptic fibrous tissue formation [1,4]. This eventually will result in the encapsulation and development of a foreign body granuloma that can be identified on imaging [4].…”

“…Such a medical situation is termed gossypiboma or textiloma. [ 47 , 48 ] The first case was described by Wilson in 1884, [ 49 ] and thereafter, an incidence rate of 1 in 300 to 1 in 3000 has been observed in past reports across all surgical procedures. Gossypiboma has been demonstrated to be associated with the development of severe clinical conditions involving various sarcomas or granuloma (pseudotumor).…”
