A call to action: Why sarcoma surgery needs to be centralized

Abstract: Although experts agree that complex surgeries or those that are performed for patients with rare conditions should be performed at high‐volume centers, the majority of patients with primary retroperitoneal sarcoma undergo surgical resection at centers with relatively low volumes. This has implications for both immediate patient outcome and long‐term clinical trial success, and highlights the importance of centralizing care for patients with such rare cancers.

“…[1][2][3] To date, these recommendations are largely supported by data demonstrating an association between surgery at high-volume centers (eg, >10-14 retroperitoneal [RP] sarcoma resections per year) and improved outcomes. [4][5][6] Although surgeon experience is important for optimal outcomes, these associations are multifactorial and not only are related to perioperative care but more likely serve as a proxy for centerwide expertise. However, when it comes to radiation therapy (RT) delivery, what constitutes a high-volume center and its impact on outcomes remain largely unanswered.…”

“…The sarcoma multidisciplinary community strongly endorses that patients with sarcoma should be managed at specialty centers given the complex and rare nature of the diseases 1‐3 . To date, these recommendations are largely supported by data demonstrating an association between surgery at high‐volume centers (eg, >10‐14 retroperitoneal [RP] sarcoma resections per year) and improved outcomes 4‐6 . Although surgeon experience is important for optimal outcomes, these associations are multifactorial and not only are related to perioperative care but more likely serve as a proxy for center‐wide expertise.…”
