A cardiovascular profile score in the surveillance of fetal hydrops

Abstract: The fetal cardiovascular profile score can be used in the surveillance of hydropic fetuses for prediction of the presence of congestive heart failure and as an aid for predicting fetal outcome.

“…It is possible that in the United States, the lack of digitization of patient records means that maternal-fetal medicine specialists have difficulty monitoring and accessing prior patient data in the manner required by modern medical science (Nielson et al, 2000). For example, if a doctor has access to EMRs they can calculate and chart an accurate fetal cardiovascular profile instantaneously for a pregnancy complicated by a dangerous condition such as hydrops (Hofstaetter et al, 2006). Accuracy of documentation may also be improved when a county moves towards electronic records: Bernstein et al (2005) finds that replacing paper obstetric records with electronic ones reduces the incidence of missing charts from 16% to 2%.…”

“…Arrhythmias also represent a leading cause of fetal hydrops (effusions in more than one fetal compartment), which has an incidence of one in 2500 pregnancies [1,4]. Hydrops fetalis is associated with up to 72% mortality, and accounts for 3% of all fetal mortality [4,5]. Digoxin is the drug of choice for treating fetal tachyarrhythmias and fetal congestive heart failure [6].…”

“…However, the relationship between fetal Doppler findings and neonatal clinical outcome has not been well established. In hydropic fetuses, the Doppler-derived fetal cardiovascular profile score (CVP) has been reported to help in the prediction of neonatal outcome 7 . We therefore tested the hypothesis that growthrestricted fetuses with adverse neonatal outcome (neonatal death or cerebral palsy) would demonstrate signs of cardiac dysfunction.…”

“…Objective To determine whether low cardiovascular profile (CVP) (28 (range, 24-35) weeks vs. 35 (range, 26-40) weeks, P < 0.01) and had lower CVP scores (4 (range,[2][3][4][5][6][5][6][7][8][9][10], P < 0.0001). All CVP subscale scores were lower (P < 0.01) in Group 1 than in Group 2 fetuses.…”
