A case of cardiac lipoma in the ventricular septum

Abstract: Lipoma in the ventricular septum is very rare. Our review of the English literature revealed that our case is the sixth of removal of lipoma in the ventricular septum. A 60-year-old male was admitted because a mass in the ventricular septum was found incidentally in abdominal CT taken following type B hepatitis. CT scanning of the heart showed 2 cm diameter of tumor in the ventricular septum. The tumor had very low radiodensity, so it was thought to be identical to fat tissue. The tumor was more clearly visual… Show more

“…Often, lipomas arise from the epicardial fat and grow into the pericardial sac [ 11 ]. However, there are case reports of distinct sites, such as the right atrium [ 10 ], left atrium [ 21 , 22 ], tricuspid valve [ 23 , 24 ], mitral valve [ 25 ], right ventricle [ 26 ], left ventricle [ 20 , 27 ], interventricular septum [ 12 , 28 ], and inside the ventricular cavities themselves [ 29 ]. Size ranges from 1 to 15 cm in diameter, and weight such as 2 kg is described [ 20 ].…”

“…The most common intracardiac localisation is in the right atrium [8–10] with a wide peduncle originating either from the septal wall or atrial roof. They have also been described in the pericardium [11, 12], in the left atrium [10, 13, 14], on the tricuspid valve [15, 16], on the mitral valve [17], in the right [10] and left ventricles [18], in the ventricular septum [19], and even in both ventricular cavities [20]. …”

“…In about 50 % of cases, lipomas are intracavitary, 25 % are intramyocardial, and the remaining 25 % extracavitary [3]. A lipoma in the interventricular septum is extremely rare and, to the best of our knowledge, there are only 7 previous case reports of lipomas located on the interventricular septum in the left ventricle [4]. Symptoms are not characteristic and depend on the lipoma's location and size.…”
