A Case of Oro-antral Communication Closed by Autotransplantation of Third Molar

Abstract: A number of surgical treatment modalities have been described in literature for closure of oro-antral communications. None of the methods however provide for immediate prosthetic rehabilitation of the communication site. We describe a case of oro-antral communication treated using autogenous third molar transplantation. At 18 months of follow-up, the transplanted tooth was functioning well with radiographic evidence of lamina dura and periodontal ligament. Its root-tip was still seen projecting into the maxill… Show more

“…Third molar autotransplantation has also been considered a promising treatment for OAF, which has many advantages, some of them being immediate restoring of a missed tooth, low cost, and possibility of orthodontic treatment. However, there is very scarce data of clinical performance in immediate postextraction oroantral communication closure and no data for chronic OAF closure [13]. So, we decided against this treatment in our case.…”

“…An unusual indication of transplantation of teeth is the use of transplanted tooth in the management of oroantral communication. Many case reports showed successful closure of oroantral communication with immediate implementation of autogenous third molar transplantation into the site of oroantral opening [ 37 , 38 ]. Transplantation of third molars with completed root formation and closed apices was also reported to be successful and promising in the management of oroantral communications [ 39 ].…”

“…A postoperative radiography examination and pulp sensitivity test were performed [24], though pulp healing was not noted [25,32,33], partly because the root development status of the tooth was stage 7 according to the Moorrees classification. Since root resorption occurs within 14 days after surgery when the need for blood revascularization is not expected [34,35], root canal therapy was performed two weeks later [8,16].…”

“…Furthermore, as compared to a partial denture, which requires a removal method, auto transplantation is a fixed method. Thus, chewing efficiency is better with auto transplantation, while it is also less expensive as compared to implants [2,3,7] Auto transplantation is considered to be a superior method for restoring missing teeth because of proprioception with the periodontal membrane [12], with other advantages known as well, including cost-effectiveness, as noted above, preservation of proprioception, possibility of a postoperative orthodontic option [14], protection of adjacent teeth, a natural prosthetic rise form from the margin, ability to be used in growing children [1,15], aesthetics [10], improvements of mastication and pronunciation [1], closure of a maxillary sinus fistula [16], maintenance of the natural shapes of the attached gingiva and interdental papillae [12], and formation of new alveolar bone [5,7,8,13].…”
