Perinealherniaisrareafterabdominoperinealresection(APR).Thelaparoscopicapproachisincreasinglybeingusedforperinealherniarepair.Wereportacaseofsuccessfullaparoscopicrepairofaperine-alherniaina69-year-old womanwhounderwentlaparoscopicAPRforrectalcancer.Thepatientwas discharged on the 22 nd postoperative day after an uneventful course. She observed perineal swelling, which only appeared while standing, approximately 1 month after the APR. Computed tomography revealedprolapseofthesmallintestineintothepelvis,andwediagnosedthepatientwithpostoperativeperinealhernia.Weperformedlaparoscopicperinealherniarepair;fourportswereinsertedtoavoidstoma sitearea.WeusedaBARD® VENTRALIGHT® STtrimmed10×15cm(3cmoverlappedforthehernia orifice).UsingEthibond,themeshwassuturedtotheposteriorsacrococcygealligament,uterus,andthe lateralpelvicwall.Norecurrenceofherniawasobservedoveroneyearpostoperatively.
