A Case of Polymicrobial, Gram-Negative Pulmonic Valve Endocarditis

Abstract: Infective endocarditis due to Gram-negative, non-HACEK bacteria is a rare clinical entity. Even moreso, isolated pulmonic valve endocarditis accounts for less than 1.5–2% of all cases of infective endocarditis. These disease pathologies commonly occur in the setting of intravenous drug abuse, indwelling catheters or cardiac devices, or underlying structural heart disease. We present a unique case of pulmonic valve endocarditis in the setting of persistent polymicrobial bacteremia with Klebsiella pneumoniae and… Show more

“…Liver abscess and urinary tract infection are the most common source when bacteremia is present 36 . Mortality rates vary depending on the pathogen, virulence factors, complications and the valve involved, reaching up to 49% in one article 6 , 37 . Urgent surgical interventions as a first-choice treatment is recommended for uncontrolled infection to prevent complications, including heart failure and embolic events 35 , 38 .…”

“…Organismos Gram-negativos estão menos relacionados a esse tipo de patologia por se incorporarem menos e com maior dificuldade nas válvulas cárdicas. A EI também pode ocorrer por infecção fúngica, acometendo em maior intensidade indivíduos que utilizam drogas injetáveis e imunossuprimidos, sendo os agentes mais prevalentes a Candida e Aspergillus spp 9 .…”
