A Case of Severe Transaminase Elevation Following a Single Ustekinumab Dose with Remission After Drug Withdrawal

Abstract: We reported a case of a 61-year old woman admitted to our Unit for severe transaminase elevation (aspartate transaminase 756 IU/l, alanine transaminase 1212 IU/l) occurred after a single Ustekinumab infusion for psoriasis. All other possible causes of liver damage, including drugs, hepatitis viruses, auto-antibodies and metabolic disorders were excluded. Liver biopsy showed lymphocytic infiltration, biliary duct damage and piecemeal necrosis. On the bases of the nonspecific histological picture and Rucam score… Show more

“…In the PHOENIX I and II studies, normal transaminase values In PHOENIX I and II, normal transaminase values were always recorded in enrolled patients [ 5 ]. A similar case was reported by Lovero R in 2018, it was a major cytolysis following UST infusion [ 6 ]. The etiological investigation was negative.…”

“…The liver biopsy showed a lymphocytic infiltration, cholangitis and piecemeal necrosis. The diagnosis retained was that of treatment-induced liver damage without HAI [ 6 ]. Spontaneous improvement of liver function after 6 months of treatment was observed without recourse to corticosteroids and without recurrence, contrary to our case [ 6 ].…”

“…In many studies, liver injury and increased transaminase values in patients treated with UST were uncommon [82]. Nevertheless, one case of severe transaminases elevation was reported in the literature [83]. Furthermore, the higher probability of developing respiratory complications with IFX, especially dyspnea, could be due to the infusion-related reactions [20].…”

“…A small retrospective study including 44 patients with psoriasis treated with ustekimumab described cases of mild elevation of liver enzymes and no cases of severe DILI[ 126 ]. Some case reports described spontaneous regression of liver injury after ustekinumab withdrawal[ 127 ].…”
