An 84-year-old Japanese woman was treated laparoscopic distal gastrectomy for early gastric cancer. Pathological examination revealed poorly differentiated adenocarcinoma limiting in mucus membrane without vessel infiltration or lymph node metastasis. An elastic soft subcutaneous tumor of right neck 1 cm in diameter was detected at surveillance one year after gastrectomy. Subsequently, many subcutaneous tumors developed on upper trunk, neck, and head. Biopsy of subcutaneous tumor revealed poorly differentiated adenocarcinoma similar to gastric cancer. Results of immunohistochemical examinations for gastric, and subcutaneous cancer were consistent with AE1/AE3, cytokeratin 7, and cytokeratin 20, and metachronous skin metastases from early gastric cancer was diagnosed. Although administration of tegafur/gimeracil/oteracil affected skin metastasis, paralytic ileus due to peritoneal dissemination developed. The patient was dead 15 months after distal gastrectomy. Skin metastasis of early gastric cancer is rare, and five cases only were reported. Review of the 5 cases and the present case suggests poor prognosis of skin metastasis despite early stage of gastric cancer. Clinicians should bear meticulous surveillance in mind to find an atypical metastasis of early gastric cancer such as skin metastasis, and to treat preemptively.
