A case of successful management of maternal septic shock with multiple organ failure following amniocentesis at midgestation

Abstract: Maternal sepsis is an unusual but catastrophic complication of amniocentesis. We report a case of successful treatment of maternal septic shock and multiple organ failure following amniocentesis at midgestation, possibly due to needle puncture of the sigmoid colon, which was tightly adherent to the anterior surface of the pregnant uterus.

“…In most cases, inadvertent bowel puncture was postulated in the discussion as a possible mechanism. A pin-hole in the sigmoid colon was actually demonstrated at laparotomy in one patient who had adhesions of bowel to the uterus [5] . Other etiologies, such as skin contamination, pre-existing subclinical chorioamnionitis, or ascending infection from the vagina have also been postulated, but never proven.…”

“…Inoculation of intestinal germs has been implicated in cases of sepsis after transabdominal procedures. In such cases, peritoneal signs are expected to predominate 69 . Incubation time is usually short, and the onset of symptoms is usually manifested within 24 hours after the procedure.…”
