A Clinical Appraisal of the Vectorcardiogram in Myocardial Infarction

Abstract: A total of 161 vectorcardiograms was recorded with the Grishman cube system in patients with suspected myocardial infarction. In the group of 49 cases subsequently examined post mortem and of 73 non-autopsied cases, in whom the diagnosis of infarction appeared certain, a comparison was made between the standard electrocardiogram and the vectorcardiogram taken in succession. The vectorcardiogram demonstrated more infarcts than the electrocardiogram. Of a total of 122 cases in which the vectorcardiogram showed i… Show more

“…Preliminary observations have indicated that the use of the VCG criteria herein proposed together with the criteria previously published, 8,9 related to the early part of the QRS st loop, results in a very high yield of positive diagnoses without overlap with values found in normals. A study is now in progress evaluating the combined use of early and late VCG QRS changes in the diagnosis of myocardial infarction in the inferior and other areas of the heart.…”

“…Superior forces for >30 ms and clockwise inscription of the first 25 to 30 ms in the frontal plane have been proposed as sensitive criteria for inferior infarction (McConahay et al, 1970;Young and Williams, 1968;Hugenholtz, Forkner, and Levine, 1961;Murata et al, 1967). It has been suggested that the vectorcardiographic appearance of inferior infarction is preserved in the presence of left anterior hemiblock (Kulbertus et al, 1971).…”

“…The diagnosis criteria for inferior lateral myocardial infarction were those of Chou et al [3] and for anteroseptal myocar dial infarction those of Hugenholtz et al [4]. The diagnosis of left anterior hemiblock and inferior myocardial infarction were accord ing to Friedman's suggestion [5].…”
