Introduction: Deep brain stimulation (DBS) has become a standard therapy for the treatment of select cases of medication refractory essential tremor and Parkinson’s disease however the effectiveness and long-term outcomes of DBS in other uncommon and complex tremor syndromes has not been well established. Traditionally, the ventralis intermedius nucleus (VIM) of the thalamus has been considered the main target for medically intractable tremors; however alternative brain regions and improvements in stereotactic techniques and hardware may soon change the horizon for treatment of complex tremors. Areas covered: In this article, we conducted a PubMed search using different combinations between the terms ‘Uncommon tremors’, ‘Dystonic tremor’, ‘Holmes tremor’ ‘Midbrain tremor’, ‘Rubral tremor’, ‘Cerebellar tremor’, ‘outflow tremor’, ‘Multiple Sclerosis tremor’, ‘Post-traumatic tremor’, ‘Neuropathic tremor’, and ‘Deep Brain Stimulation/DBS’. Additionally, we examined and summarized the current state of evolving interventions for treatment of complex tremor syndromes. Expert c ommentary: Recently reported interventions for rare tremors include stimulation of the posterior subthalamic area, globus pallidus internus, ventralis oralis anterior/posterior thalamic subnuclei, and the use of dual lead stimulation in one or more of these targets. Treatment should be individualized and dictated by tremor phenomenology and associated clinical features.

Essential tremor is an action tremor syndrome of upper limbs lasting at least three years, which may be accompanied by tremor in other body parts and subtle neurological signs. For long considered benign, its association to non-motor symptoms and neurodegenerative diseases has been demonstrated in the literature. In addition, the perception of heterogeneity in the behaviour of the disease, with different characteristics between patients of early-and late-onset tremor, is strengthened. An increased risk of Parkinson's disease is known among patients diagnosed with Essential Tremor. The diagnosis of Parkinson's disease is based on motor symptoms, with a prodromal phase for which diagnostic criteria were published relying on risk factors of the disease and prodromal markers. However, in literature, there is a need for further exploration of the behaviour of the prodromal phase of Parkinson's disease and its symptoms in patients with Essential Tremor. Objective: To evaluate the prevalence of risk factors and prodromal markers of Parkinson's disease and to diagnose their prodromal phase in groups of elderly patients with Essential Tremor of late-and early-onset, assessing whether late-onset ET patients have a higher prevalence of symptoms and prodromal Parkinson's disease. Methodology: A crosssectional, case-control study included patients with diagnosis of Essential Tremor and Essential Tremor Plus aged 60 or more, without parkinsonism and with cognitive performance within the norms standardized in the Mini Mental State Examination. These patients had their follow-up at the Extrapyramidal Diseases Outpatient Clinic of the Medical School of Ribeirão Preto. 43 patients were divided into two groups, one of patients with Essential Tremor of early-onset (before 60 years of age) and another of patients with late-onset of symptoms (after 60 years of age). The groups were composed respectively of 30 and 13 patients. All patients underwent an evaluation to collect sociodemographic, general clinical data and to assess the presence of risk factors and prodromal markers of Parkinson's disease. The data were later analyzed for the presence of prodromal Parkinson's disease according to the methodology proposed by the Movement Disorders Society. Results: The group of patients with late-onset Essential Tremor was composed of older patients (p = 0.039) with a shorter average duration of the tremor (p <0.001). No statistically significant difference was observed between the groups regarding the frequency of risk factors and prodromal markers of Parkinson's disease. The diagnosis of prodromal Parkinson's disease was possible in only one patient with early-onset Essential Tremor. Conclusion: No statistically significant difference was observed between the groups regarding the frequency of risk factors and prodromal markers of Parkinson's disease.
