Insertion of laryngeal mask airway has been facilitated by using a variety of induction agents and their combinations with minimal side effects. The current prospective study is a randomized, double-blind study conducted using induction agents, namely, propofol and its equipotent dose of thiopentone, for laryngeal mask airway insertion, and to compare their side effects in patients undergoing minor surgeries requiring general anaesthesia. MethodsThis prospective study was carried out at the Anaesthesiology and Critical Care Department of Gauhati Medical College and Hospital (GMCH), Assam, India. The randomized, double-blinded study comprised 80 patients aged 18 to 60 years undergoing minor surgeries (≤45 minutes) under general anaesthesia fitting into the American Society of Anesthesiologists (ASA) physical status I and II and Mallampati score (MPS) 1 and 2. The participants were randomly divided into two groups in a 1:1 ratio. Group A (n = 40) received propofol (2.5 mg/kg), while group B (n = 40) received thiopentone (5 mg/kg) injections for induction of anaesthesia. Pre-medication with midazolam (0.04 mg/kg) injection and fentanyl (1.5 mcg/kg) injection was provided to patients in both groups. Post-laryngeal mask airway insertion, parameters like conditions for insertion, time taken for laryngeal mask airway insertion, overall response, and haemodynamic parameters were recorded. The data analysis was executed using equivalence tests considering a two-sided p < 0.05 as significant. ResultsGroup A had a higher and statistically significant ease of insertion (p = 0.029). The mean insertion time was notably different between the two groups (p < 0.001). The difference in the overall response to insertion showed no statistical significance in the two groups. Statistically, a significant difference was found in falls in heart rate and various blood pressure levels between the groups (p < 0.001). ConclusionPropofol at a rate of 2.5 mg/kg was found to be superior to thiopentone at a rate of 5 mg/kg as far as suppression of upper airway reflexes in laryngeal mask airway insertion.

Aims: The aim of the study is to compare the conditions to facilitate the insertion of the laryngeal mask airway (LMA) with the two most commonly used agents – thiopentone and propofol, after adequate pre-medication with midazolam and fentanyl. Methods: It is prospective randomized study conducted on 70 patients of either sex, belonging to 20–60 years of age, and American Society of Anesthesiologists grades I and II who were admitted to elective minor surgeries under general anesthesia. Patients were randomly divided into 2 groups. Patients in both the groups received pre-induction doses of glycopyrrolate (0.2 mg), midazolam (0.04 mg/kg), and fentanyl (2 mcg/kg) and were induced with either propofol (2 mg/kg) or thiopentone (5 mg/kg), to facilitate insertion of LMA. The parameters were studied and were conditions for insertion, time taken, hemodynamic changes during insertion of LMA, and immediately thereafter. Results: Conditions for insertion, ease of insertion was significantly greater in propofol group when compared to thiopentone group. The difference was statistically significant (p<0.05). The heart rate, systolic blood pressure, and diastolic blood pressure were compared between the 2 groups. It was observed that there was a fall in all these parameters in both groups, but it was more pronounced in the propofol group. Although this was statistically extremely significant (p<0.001), it was not clinically significant. Conclusion: It was concluded that, due to better ease of insertion, lesser time taken for insertion, and better recovery profiles observed with propofol, it seemed to be slightly superior to thiopentone as an induction agent for insertion of the LMA.

Background: This study aimed to compare the efficacies of four anesthetic induction drugs (thiopental, propofol, midazolam-thiopental, and etomidate-propofol) on cardiovascular response during laryngeal mask airway (LMA) placement in eye surgery. Materials and Methods: The present clinical trial study included 128 patients who were candidates for ophthalmic surgery in four groups. Patients in the first group were given a combination of midazolam (0.04 mg/kg) with thiopental (2.5 mg/kg) (Group T + M). We administered propofol alone (2.5 mg/kg) to patients in the second group (Group P). The third group received a combination of etomidate (0.1 mg/kg) with propofol (1 mg/kg) (ET + P group) and patients in the fourth group received thiopental drug (5 mg/kg) alone (Group T). Then, the stability of patients' hemodynamic parameters before anesthesia was evaluated and compared immediately after anesthesia, 1, 3, and 5 min after LMA placement. Results: There was no significant difference between the four groups in changes in oxygen saturation level (P > 0.05). Furthermore, the difference between decreased systolic blood pressure and diastolic blood pressure over time was not significant in 5 min in both Groups T + M and T (P > 0.05). In addition, the stability of these two groups was higher than the other two groups (P < 0.05) and the most unstable group was Group P. The changes pulse ratein the P group were significant (P < 0.05). Conclusion: According to the results of the current study, thiopental and Midazolam can be used as an effective induction compound to facilitate LMA insertion with higher hemodynamic stability compared to propofol alone, propofol and etomidate, and thiopental alone.
