We report an analysis of 3 single-piece hydrophobic acrylic intraocular lenses (IOLs) that were explanted because of complications related to the presence of their haptics in the ciliary sulcus. In 2 cases, the IOL was primarily implanted in the ciliary sulcus because of inadequate posterior capsule support. In the third case, postoperative displacement of 1 haptic in the sulcus was associated with hyphema and vitreous hemorrhage. Ultrasound biomicroscopy confirmed the contact between the haptic and iris. Areas of iris atrophy were observed in all cases. Light microscopy and scanning electron microscopy of the explanted lenses revealed the presence of pigmentary dispersion on the anterior surfaces. In Case 3, the pigments were concentrated on the surface of the haptic that was displaced from the capsular bag and on the corresponding optic-haptic junction. Scanning electron microscopy also showed the IOL's squared edges and unpolished side walls. The flexibility and thickness of the single-piece hydrophobic acrylic haptics, as well as the square optic and haptic edges, may increase the risk for iris chafing when the haptics are in the sulcus. Therefore, sulcus fixation of this IOL design is not recommended.

Objetivo: Avaliar os benefícios de uma técnica alternativa de capsulotomia anterior, criada para garantir a completa implantação da lente intra-ocular no saco capsular, durante a extração extracapsular da catarata. Métodos: Cento e nove olhos foram operados por meio dessa técnica e acompanhados durante o período de 1 ano. As possibilidades da técnica foram avaliadas em relação a dois aspectos: a garantia de perfeita colocação da lente no saco capsular e a capacidade de promover eficiente fixação do implante, por meio das características apresentadas pelas porções remanescentes da cápsula anterior. O primeiro aspecto foi analisado considerando o número de casos em que as duas abas puderam ser adequadamente observadas durante a implantação. O segundo aspecto foi avaliado em função do posicionamento da lente, 1 ano após a cirurgia. Dificuldades e complicações específicas dessa técnica foram também, investigadas. Resultados: Durante a implantação, as duas abas puderam ser adequadamente identificadas em 96 (90,6%) casos. Um ano após a cirurgia, a lente se encontrava centralizada em 81,9% dos casos, discretamente descentralizada (menos de 1 mm), em 13,3% e descentralizada (mais de 1 mm) em 4,8%. Conclusões: Esses resultados, quando comparados aos apresentados em relação a outros tipos de capsulotomia, indicam que essa técnica é boa alternativa para promover a adequada implantação da lente no saco capsular, na extração extracapsular da catarata. No entanto, nem sempre foi tão simples assim. Houve época em que terminar a cirurgia com toda a lente atrás da íris já era um grande feito. Pouco importava em que parte da câmara posterior suas alças viriam a encontrar apoio. Colocá-las no saco capsular, ou no sulco ciliar, dependia quase que exclusivamente do acaso (1)(2)(3) . Na verdade, o que mais comumente ocorria, era acabar com uma delas no interior do saco e a outra, fora dele (4)(5)(6)(7) . Drug-Induced Secondary Cataract Prevention: Experimental ex vivo and in vivo Results with Disulfiram, Methotrexate and Actinomycin D

Background/Aims: A clinical approach to prevent secondary cataract after lens implantation involves the intraocular application of pharmacological agents. The goals of our study were to develop an ex vivo model to test the drug effectiveness for lens epithelial cell ablation from the basal membrane and to verify the data in rabbit intraocular lens implantation experiments. Methods: Human capsular rhexis specimens were incubated with drugs and the residual cells were differentiated by use of the Live-Dead assay and quantified by staining with Hoechst dye. After phakoemulsification of rabbit eyes and before intraocular lens implantation, capsular bags were filled with drug-loaded hyaluronic acid for 5 min. Results: An ex vivo model was established which allows the testing of drugs on lens epithelial cell ablation. Drug treatment reduced the number of viable cells on the specimens drastically, ranging between 0.44 ± 0.53% (6.0 ± 7.3 cells/mm²) for disulfiram, 0.27 ± 0.50% (3.7 ± 6.9 cells/mm²) for methotrexate and 0.07 ± 0.19% (0.1 ± 0.27 cells/mm²) for actinomycin D. Rabbit eyes treated with a mixture of methotrexate/actinomycin D showed no posterior capsule opacification at 4 months and a low opacification 6 months postoperatively. Without drug treatment low opacification starts 6 weeks postoperatively. Conclusions: The drug screening in the described ex vivo model can help to reduce the number of preclinical studies for secondary cataract prevention. The successful ex vivo cell ablation by methotrexate/actinomycin D was confirmed by a delayed in vivo secondary cataract formation.
