ABSTRACT:Enterococcus faecalis is one of the leading causes of many infections and mainly urinary tract infections. This pathogen developed high resistance to multiple antibiotics and it harbor many virulence factors genes. This study aimed to determine the antibiotic resistance patterns and screening for some virulence factor genes of E. faecalis isolated from urinary tract infection. Urine samples were collected from 788 outpatient's clinic having clinical signs of UTI that visited Azadi Teaching Hospital in Duhok city. Urine samples were cultured on bacteriological media and isolated colonies identified using standard bacteriological methods. Antibiotic susceptibility was performed by Kirby Bauer test. All isolates were subjected to species-specific PCR assay for confirmatory identification followed by targeting virulence genes. Twenty five isolates of E. faecalis were detected and confirmed by species-specific PCR assay that expressed high antibiotic-resistance to many selected drugs except norfloxacin, penicillin and ampicillin. The most prevalent genes among all isolates were cpd genes followed by asa1, ace, esp, and gelE. Bearing of virulence genes combination were more frequent among multiple-antibiotic resistant strains. This study highlighted on E. faecalis as causes of UTI in Duhok city that showed multiple resistances to common antibiotics and harboring more than one virulence gene.

Objective: This study aimed to identify antibiotic susceptibility rates of enterococcal strains, and to compare the high-level resistance to aminoglycosides (HLAR) in vancomycin-sensitive enterococcal species (VSE) and vancomycin-resistant enterococcal species (VRE). Methods: The study included 100 VRE and 100 VSE strains recovered from the samples sent to laboratory from various departments of Haydarpaşa Numune Training and Research Hospital. Results: All VRE strains were defined as Enterococcus faecium, although of the VSE strains, 53% were identified to be as Enterococcus faecalis, 42% E. faecium, 3% Enterococcus durans, and 2% Enterococcus avium. High-level resistance to vancomycin (MIC, >256 µg/ml) was determined in all VRE strains and when analyzing MIC values for teicoplanin, five strains were found to be moderately susceptible (MIC, 16 µg/ml) and 95 strains were resistant (MIC, >32 µg/ml). Of the VRE strains, one was linezolid-resistant (MIC, 12 µg/ml) and the other was intermediately susceptible (MIC, 4 µg/ml) and remainders were evaluated to be susceptible (MIC, <2 µg/ml). In VRE strains, high-level gentamicin resistance (HLGR) was found to be 83% and high-level streptomycin resistance (HLSR) 89%, association of HLSR with HLGR was 78%. In VSE strains, HLGR was found to be 42% and, HLSR 48%, the association of HLSR with HLGR was found to be 36%. HLAR in VRE strains was found to be higher as compared with VSE strains (p <0.005). Conclusion: Antimicrobial resistance is increasing in enterococci strains. Therefore a follow-up is required resistance pattern including both vancomycin resistance and HLAR. Occurrence of antimicrobial resistance and virulence genes, and distribution of enterococcal clonal complex 17 from animals and human beings in Korea

Abstract. Enterococci are major zoonotic bacteria that cause opportunistic infections in human beings and animals. Moreover, pathogenic strains can be disseminated between human beings and animals, particularly companion animals that come into frequent contact with people. Recently, Enterococcus faecium clonal complex 17 (CC17) has emerged as a pandemic clone. Most CC17 strains are ampicillin resistant and possess virulence genes such as esp and hyl. Despite the possible dissemination of CC17 between human beings and animals, prevalence data about CC17 in animals is limited. In the present study, the phenotypes and genotypes of antimicrobial resistance were compared, as well as virulence gene profiles from 184 enterococci strains isolated from chickens, pigs, companion animals, and human patients in Korea. Ampicillin-resistant E. faecium (AREF) strains were selected, and multilocus sequence typing was performed to investigate the dispersion of CC17 among animals and human beings. The companion animal and human isolates showed high resistance rates to ampicillin and ciprofloxacin, whereas food animal isolates showed high tetracycline and erythromycin resistance rates. Ampicillin-resistant E. faecium was only detected in human (21/21 E. faecium, 100%) and companion animal (3/5 E. faecium, 60%) isolates, and all human AREF strains and 1 canine AREF strain were confirmed as CC17. In conclusion, the occurrence of antimicrobial resistance and virulence genes, and the distribution of enterococcal CC17 in companion animal enterococcal strains were similar to those of human strains rather than to those of food animal strains.
