A comparison of knee braces during walking for the treatment of osteoarthritis of the medial compartment of the knee

Abstract: In this cross-over study, we evaluated two types of knee brace commonly used in the conservative treatment of osteoarthritis of the medial compartment. Twelve patients confirmed radiologically as having unilateral osteoarthritis of the medial compartment (Larsen grade 2 to grade 4) were studied. Treatment with a simple hinged brace was compared with that using a valgus corrective brace. Knee kinematics, ground reaction forces, pain and function were assessed during walking and the Hospital for Special Surgery … Show more

“…One RCT compared 2 different types of valgus knee braces (custom-fit versus off-the-shelf) and was excluded from the meta-analysis (39). Another RCT provided insufficient pain and function data for inclusion in the meta-analysis (40). Both RCTs were included in the description of instructions, complications, and compliance ( Figure 1).…”

“…ported the brace to be bulky, but did not include the number of patients; therefore, an event rate could not be calculated (40).…”

“…Correction of malalignment was evaluated with a standardized standing whole-leg radiograph. Although both groups (valgus brace and wedged insole) had improved patient-reported outcomes, no significant change in alignment was seen on this static evaluation [26], so whether a brace or insole corrects malalignment in the frontal plane remains controversial [8,15,21,23,26].…”

“…This can be treated conservatively with various unloading braces for the knee. 2,3 However, the compliance with these devices can be problematic due to the fact that they are perceived as being cumbersome to implement on a daily basis. 4 Another effective and economical treatment option for medial knee OA is the laterally wedged foot orthotic device.…”
