A Comparison of VerifyNow® with PlateletMapping® -Detected Aspirin Resistance and Correlation with Urinary Thromboxane

Abstract: These results indicate that PlateletMapping could be a useful point-of-care assay to identify aspirin-resistant patients for better perioperative risk stratification and management.

“…This finding of normal platelet function and lack of inhibition by TEG-PM testing raises two pertinent questions regarding (1) the sensitivity and ability of TEG-PM to evaluate platelet inhibition in the setting of ASA and (2) the ability of ASA to provide lasting platelet inhibition in children requiring prophylaxis prior to surgery. The TEG-PM has been shown to demonstrate good correlation with urinary thromboxane B2 levels 11,38 and with light transmittance platelet aggregation (LTA). 11,39 The LTA 40 and urinary 11dehydrothromboxane B2 levels 41 are considered to be the two standards in the evaluation of ASA responsiveness via the AA pathway.…”

“…The TEG offers an assay called PlateletMapping®, FDA-cleared, that uses maximum amplitude to detect ALR and CLR. 22 ROTEM assays for ALR and CLR await FDA clearance. Diagnostica Stago, Inc, offers the research use only intracellular vasodilator-stimulated phosphoprotein (VASP) phosphorylation enzyme immunoassay, Cy -Quant VASP/P2Y12.…”

“…PM shows good sensitivity for detecting aspirin resistance and has been used to demonstrate that reversal of platelet inhibition by aspirin or clopidogrel (Plavix) begins about 3 days after stoppage [Carroll et al., ; Cattano et al., ]. Mahla et al.…”

“…Verify Now has agonists to examine the antiplatelet activity of GPIIb/IIIa inhibitors, ASA, and clopidogrel, and is able to report and quantify the degree of platelet inhibition with good correlation to laboratory‐based LTA [Malinin et al., ; Gremmel et al., ; Grove et al., ]. The system may be less sensitive than Platelet Mapping for assessment of aspirin resistance [Carroll et al., ].…”
